- This is a combined synopsis/solicitation for commercial item prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITION; QUOTATIONS ARE BEING REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED. Response shall reference ?Request for Quotation NO. DE-RP04-02AL67785.? This combined synopsis/solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-02. This procurement is anticipated to be awarded under full and open competition; however, we reserve the right to award on a 100% Small Business Set-Aside. The recommended North American Industry Classification System (NAICS) for this procurement is 561790, with a corresponding size of $5M (indicates maximum allowed for a concern including its affiliates, to be considered small). Offerors responding should indicate whether or not they are a small business, small woman-owned business, small disadvantaged business or 8(a). This will be a Firm-Fixed Price solicitation. Work shall be performed during non-standards hours. DOE-AL intends to acquire a competitive price on the following: DOE/AL Complex HVAC/Duct Cleaning. 1) Clean the HVAC air conveyance systems in all designated buildings. Approximately: 17 buildings ? estimated 289,028 sq. ft. 2) Special Requirements: Contractor must develop and submit a work plan that describes the duct cleaning process. Include a project schedule that indicates site mobilization, mechanical and electrical shutdowns and isolations, pre cleaning activities, cleaning activities, post cleaning activities and repair restoration of the system. 3) Develop and submit for DOE approval a site specific preliminary hazard assessment (PHA) plan that addresses both existing site hazards and hazards introduced by the duct cleaning operations. Attach Material Safety Data Sheets for all chemical brought on site. 4) Develop and submit for DOE approval a Medical and Emergency Plan. 5) Mechanical Drawings can be reviewed during the initial walk thru. These drawings are to be maintained in accordance with UNCI, as defined in Section 148 of the Atomic Energy Act of 1954, as amended. UNCI may on be made available to authorized individuals for the purpose of this contract, means only to U.S. Citizens who have a need to know in the performance of official duties or DOE authorized activities and who are employees of the government. 6) General Specifications for the cleaning of heating, ventilating and air condition systems will be provided to contractor during pre-conference meeting. 7) Contractor will also be required to provide driver licenses for personnel scheduled to work on project and must provide auto insurance and vehicle information. Delivery shall be FOB Destination. All commercial provisions and FAR clauses referenced in this solicitation are available in full text at the following web site location: http://www.arnet,gov/far/. FAR 52,212.1, Instructions to Offerors Commercial applies to this acquisition. Only those Offerors who can furnish the items as per specifications provided herein will be considered. Award will be made on the basis of lowest overall price. This is an ?All or None? solicitation and Offerors must propose on all items. Offerors shall include a completed copy of the provisions at FAR 52.212.3, Offeror Representations and Certifications?Commercial Items with the offer. The clauses at FAR 52.212-4 Contract Terms and Conditions?Commercial Items, including addenda: DEAR 952.202-1, Definitions apply to this acquisition. In addition, FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders?Commercial Items, applies to this acquisition. Under FAR 52.212-5, paragraph (b), the Contracting Officer has indicated that the following clauses are incorporated by reference: FAR 52.22-21, Prohibition of Segregated Facilities; FAR 52.222-26, Equal Opportunity; FAR 52.222-35, Affirmative Action for Disabled Ve terans and Veterans of the Vietnam Era; FAR 52.222-36, Affirmative Action for Workers with Disabilities; FAR 53.333-37, Employment Reports on Disabled Veterans and Veterans of the Vietnam Era; FAR 52.222-38, Compliance With Veterans?
