Description
This synopsis/solicitation is for commercial items prepared in accordance with the format in FAR Subpart 12.6, utilizing part 15, negotiations techniques, and as supplemented, with the additional information included in this notice. Solicitation number RFP 546-44-02; award will be made using the evaluation factors identified in the solicitation. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-7. This acquisition is Set-Aside 100% for Small Business. The NAICS is 532291 and the small business size standard is 6.0 million. The acquisition is for Durable Medical Equipment (DME). Requirement: 1. The purpose for this solicitation is to award a contract for durable medical equipment (DME). The Contractor shall deliver, pick up, provide, install, service, repair, and provide storage for durable medical equipment to eligible VA patents as required by the VA Medical Center in Miami and West Palm Beach. The Contractor must possess all licenses, permits, accreditation and certificates as required by the State of Florida, in addition the selected firm shall be Joint Commission Accredited (JCAHO). 2. The Contractor must provide documentation that they have been in the durable medical equipment business for a minimum of at least three (3) years and have had a physical location which houses the service office, warehouse, dispatch of vehicles and other functions related to the performance of services within the ten (10) county cachment area. Facilities must be ADA compliant. Cachment areas for West Palm Beach include: Palm Beach, Martin, Okeechobee, Glades, Indian River, Hendry and St. Lucie Counties. Cachment areas for the Miami VA include: Broward, Dade and Monroe counties. 3. Contractor will service and supply patient's residences located in the ten (10) county cachment areas referenced above. 4. Contractor must make available all equipment and supplies to any patient in any area. 5. Contractor shall provide service, twenty-four (24) hour per day, seven (7) days a week, 365 days a year. Contractor shall respond within 48 hours of notification for service by VAMC, within a one hundred (100) mile radius of the Miami VAMC and the West Palm Beach VAMC. Emergencies shall be supported within twenty-four (24) hours. This resulting contract will be for a period of one year, with the option to extend for 2 additional one year periods.
