Description
The VA Desert Pacific Healthcare Network (VISN 22), Network Business Center (NBC), Long Beach, CA, is seeking to establish a turnkey Capitated Rate Primary Care Community-Based Outpatient Clinic (CBOC) facility in South Central Los Angeles, CA for VA Greater Los Angeles Healthcare System (GLA) located at 11301 Wilshire Blvd, Los Angeles, CA 90073. The turnkey facility shall also provide two offices for GLA-staffed Mental Health Clinic services. The qualified contractor (see note below) shall provide all clinical space and support, all outpatient primary and urgent care services at an all-inclusive rate. GLA patients shall have scheduled follow-up clinic visits that shall be scheduled by the qualified contractor utilizing the GLA computer system. GLA shall provide training for proper utilization of the scheduling modules. The qualified contractor may see new referrals or unscheduled GLA patients, under the contract, once eligibility is established, when under tentative eligibility as authorized by the GLA, or when, upon consultation with the GLA, approval is given. The clinic schedule shall be consistent with GLA?s workweek Monday through Friday, 8:00 a.m. to 4:30 p.m., excluding federal holidays. Designated contact telephone numbers shall be provided from GLA facility for both clinic staff and GLA patients to use. Primary care is generally defined as the delivery and management of primary and preventative medical care and the continuity of that care. At a minimum, Primary Care services provide intake, initial assessment, health promotion, disease prevention, management of acute minor illnesses and chronic bio-psychosocial conditions, pharmacotherapy management, physical examinations, primary care women?s health, injections and immunizations, referrals for specialty, rehabilitation, and other levels of care, follow-up overall care management, and patient and caregiver education. Based on 1990 US Census data, veteran population in South Central Los Angeles is estimated to be 50,000 with estimated 3,000 area veteran patients presently enrolled at GLA. The minimum guarantee is 100 patients. The qualified contractor would provide clinical providers, medical facilities for Primary and Urgent Care, medical equipment and supplies and all administrative functions to fulfill all levels and ranges of care expected for an initial one-year period with four (4) subsequent one-year option renewals. All requests for a solicitation package shall be addressed to the NBC Contracting Officer in writing using your company letterhead and received by 4:00 p.m. PDST on July 15, 2002. Fax requests for a solicitation package shall also be accepted at NBC fax number 562.826.5828. The company letterhead must contain a company contact person, telephone and fax numbers, and complete address. Telephone requests for a solicitation package shall not be accepted. The anticipated solicitation issue date is approximately on or about August 9, 2002. The North American Industrial Classification System (NAICS) Code is 621498. Small business and small disadvantaged businesses are highly encouraged to participate. VA shall not be responsible for any cost associated with your request for this solicitation. Note: All usage of the term "qualified" in reference to personnel and procedures includes reference to a level of service consistent with current requirements to meet and maintain accreditation by the Joint Commission for Accreditation of Hospital Organizations (JCAHO) and that fully meet the standards established by the GLA Medical Executive Board
