- Description
- This is a commerical item solicitation using FAR Part 12 and 15. The VA Salt Lake City Health Care System, Salt Lake City, Utah has a requirement for a Community Based Outpatient Clinic (CBOC) to cover primary care services in Duchesne, Uintah, and Daggett counties in the State of Utah. The contractor shall provide for the delivery and management of continuous primary and preventive medical care for eligible VA patients. The contractor will provide health care clinical providers, medical facilities, medical equipment, supplies, laboratory and imaging services, pharmacy services and all administrative services. Services will be at a capitated rate and will be all inclusive. The period of performance will be for a base year from October 1, 2002 through September 30, 2003 with four one year options to extend for a total of five years. The NAICS Code is 621999.
