Description
VA is seeking proposals from Contractors able to provide all labor, supervision, and management to delivery Home Health Services that promotes the finest quality of life for patients enrolled for benefits from the Department of Veterans Affairs (VA), Mid South Veterans Healthcare Network (VISN 9). Full services shall be provided to motivate and educate patients to achieve and maintain independence in the activities of daily livings (ADL). Every effort shall be made to keep patients ambulatory, reduce hospitalizations, and to achieve an optimal level of care. At this time VA is awarding multiple, performance-based contracts for Homemaker and Home Health Aide services. However, Contractors that also offer skilled nursing and hospice services are requested to provide pricing for all the services they can provide. The initial requirement is for services for veterans living in the geographical area under the responsibility of the Memphis VA Medical Center. That area includes, Western Tennessee, Northern Mississippi, and Eastern Arkansas. VISN 9 is responsible for all of Tennessee, and sections of Kentucky and West Virginia. VISN 9 Medical Centers are located in Huntington, WV, Lexington, KY, Louisville, KY, Memphis, TN, Johnson City, TN, Murfreesboro, TN, and Nashville, TN. The veteran population serviced by those facilities cover large rural outlying areas. Contractors shall include in their price proposals the definition of the geographical areas they serve. Contractors can offer proposals for any combination or all locations and skill levels. Initially there will be 2 or 3 awards for services in Western Tennessee, 1 for Eastern Arkansas, and 1 for Northern Mississippi. Additional geographical areas may be added for the other VISN 9 facilities. Contractors must be accredited by the Joint Committee for Accreditation of Healthcare Organizations (JCAHO) and Medicare.
