- NA This is a combined synopsis/solicitation for commercial items prepared in accordance with the format used in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals a re being requested and a written solicitation will not be issued. DAHA25-02-T-0022 is being issued as a Request for Quotations (RFQ). This document and incorporated provisions and clauses are those in effect with Federal Acquisitions Circular 01-07. This combined synopsis/solicitation has been set-aside 100% for small businesses. The SIC Code is 3577. 17 Inch .27MM SVGA Monitors with 1280x1024 Resolutions and Energy Star Compliant. Quantity of 500 each. Delivery to Lincoln, Nebraska, Computer Rebuild & Repair, 2960 North Park Road, Lincoln, NE 68524. Delivery of all 500 monitors no later than July 25, 2002. Partial delivery of at least one-third may be done and partial payments will be authorized. Please call 402-309-1920 for all technical questions. Please do not call Sandy Leach for any technical questions. If you wish to e-mail questions to her she will forward to the end users. Any and all shipping costs must be included in the pricing of the monitors and not as a separate charge. If vendor qu ote lists shipping as a separate charge, vendor quote will be considered non-responsive. The following FARS are applicable to the combined synopsis/solicitation, however, please do not send them at this time, send only pricing. These FARS are: 52.212-1, Instructions to Offerors-Commercial Items; 52.212-2, Evaluation-Commercial Items (lowest price that meets the specifications); 52.212-3, Offeror Representations & Certifications-Commercial Items; 52.212-4, Contract Term & Conditions-Commercial Items; 52.21 2-5, Contract Terms & Conditions Needed to Implement Statutes on Execitive Orders-Commercial Items; and 52.247-34, FOB-Destination. All quotes must be sent via e-mail to sandra.leach@ne.ngb.army.mil no later than 4:00 p.m. CDT time, Thursday, June 27, 200 2. Mandatory information required in your quote is: Solicitation Number of DAHA25-02-T-0022, Vendor Name and Address, Vendor Phone and Fax Number, Vendor Point of Contact, and Vendor Duns & Bradstreet Number. If any or all of this information is not incl uded in the vendor quote, the vendor quote will be considered non-responsive. All vendors/prospective contractors must be registered in the CCR database (www.ccr.gov) prior to award date of Friday, June 28, 2002. If not registered or active as of 7:30 a. m. CDT on Friday, June 28, 2002, vendor quote will be considered non-responsive. Once again only e-mail quotes will be accepted. No faxed quotes accepted for any reason.
