- Description
- This is a combined synopsis/solicitation for Commercial Items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is IFB672-007-02, and the solicitation is issued as an invitation for bid (IFB). The solicitation document and incorporated provisions are those in effect through Federal Acquisition Circular 2001-07. This procurement is 100% Small Business set-aside. The associated NAICS code is 446199 and the Small Business Size Standard is $6.0. Provide bids for the following line items: (1) IVUS (INTRAVASCULAR ULTRASOUND SYSTEM) 1 ? EA $_________. (2) FX IVUS IMAGING CATHETER 2.9 f ? 1 EA $__________. (3) MANUAL MAINTENANCE ? 1 EA $AT NO CHARGE. Offerors shall submit all technical questions regarding this solicitation to the Contracting Officer, in writing, no later than 7 days after issuance of this Request for Proposal (RFP). Questions may be sent via facsimile, or via e-mail at rene.esteves@med.va.gov and elisa.cruz@med.va.gov (please send e-mail questions to both addresses. Verbal questions will not be addressed. Items shall be delivered by or before 07/31/2002, at the Department of Veterans Affairs Medical center, San Juan, Puerto Rico 00921-3201, FOB destination. The following provisions are applicable to this acquisition: (1) FAR 52.212-1, Instructions to Offerors-Commercial, (2) FAR 52.212-3, Offeror Representations and Certifications-Commercial Items, a completed copy of which shall be included with the offer. The following clauses are applicable to this acquisition: (1) FAR 52.212-4, Contract Terms and Conditions-Commercial Items, (2) FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items: (a) The Contractor shall comply with the following FAR clauses, which are incorporated in this contract by reference, to implement provisions of law or executive orders applicable to acquisitions of commercial items: (1) 52.222-3, Convict Labor (E.O. 11755). (2) 52.233-3, Protest after Award (31 U.S.C. 3553). (b) The Contractor shall comply with the FAR clauses in this paragraph (b) that the Contracting Officer has indicated as being incorporated in this contract by reference to implement provisions of law or Executive orders applicable to acquisitions of commercial items or components: [X] (ii) Alternate I of 52.247-64 Sealed, written offers shall be referred to: VETERANS AFFAIRS MEDICAL CENTER, 10 CALLE CASIA, SAN JUAN, PR 00921-3201 ?
