Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation Part 12. This announcement constitutes the only solicitation; proposals are being requested and a WRITTEN SOLICITATION WILL NOT BE ISSUED. Solicitation number F25600-02-T 0087 is issued as a request for quotations. 55 CONS intends to issue a purchase order for Portable Restrooms (rental and servicing) Statement of Work 1. Scope: The work consists of furnishing all equipment, labor, and materials in performing all operations in connection with this service contract. 2. Major Item of Work: a: Furnish and maintain portable restrooms according to the attached schedule (technical exhibit 1). All restrooms provided by the contractor, except for those restrooms supplied for the Offutt AFB open house, shall be brown in color. All restrooms shall be new or like new conditions. Each restroom shall have translucent panels in the top or some other approved means of allowing occupants to see with the door closed. Each restroom shall have a service record affixed to an interior wall on which the contractor shall keep a record of the date when the restroom is serviced. All restrooms shall be secured in place to avoid any tipping over. The contractor shall be required to right any restrooms, which are tipped over. b. The contractor shall furnish and maintain one hundred (100) standard and six (6) Handicapped portable restrooms for Offutt AFB open house for three (3) days (those dates will be announced by the Wing commander. These restrooms shall be in place by 10:00 AM the day prior to the open house. The contractor shall service the restrooms throughout the three days as necessary to maintain a sanitary condition. This will require at a minimum one individual and one service pump truck to be on Offutt AFB for the entire operating hours of the air show. The typical hours of operation are 9:00 AM to 5:00 PM. Servicing of restrooms can be accomplished from 0600 hrs until 0900 hrs prior to air show, or from 1700 hrs until 2100 hrs after air show c. All restrooms are indicated on technical exhibit 1 shall be pumped/emptied weekly by the contractor on the day specified. The contractor shall disinfect, sanitize, and recharge holding tanks with a sustaining deodorizer agent. The contractor shall supply a sufficient amount of toilet paper for each restroom to last for one week between cleanings. The contractor shall also clean the inside and outside of the restrooms on a weekly basis, this shall include the walls, floors, roof, and all toilet fixtures. After cleaning the restrooms shall be free of dirt, stains, and spider webs. d. The contractor shall ensure that all the base lake units be pumped/emptied, cleaned, and re-supplied with toilet paper on July 3rd. e. The contractor shall furnish twelve (12) each additional restroom at the base lake area the 2d week in June thru the 4th of July. Servicing of these additional restrooms shall be in the same manner as the other restrooms in place. f. The contractor shall coordinate the placement and position of all restrooms with the Contracting Officer's designated representative. 3. RECEIPT OF SERVICES: Invoices will be supported by signed service/delivery tickets proving actual services rendered. Invoices will include the name of contractor, order number, date of service rendered, itemized list of services actually rendered, and total amount due for the period covered. All invoices will be forwarded to 55th CES/CEOEM, 106 Peacekeeper Drive, Suite 2N3, Stop Code 4109, Offutt AFB, NE, 68113-4019. This solicitation is issued as a 100 percent small business set-aside; the NAICS Code is 562991; the NAICS size standard is5 MILLION; and the SIC Code is 7359. All potential bidders are reminded, in accordance with DFARS 252.204-7004, Required Central Contract Registration, that lack of registration in the CCR database BY AWARD DATE will make a bidder ineligible for award. NO PARTIAL QUOTES WILL BE CONSIDERED. NO PARTIAL AWARDS WILL BE MADE.
