NA AMENDMENT 0002 TO DADA16-02-R-0003 IS TO: (1.) Section -1, 1.1. Scope of Work. Standard. DELETE: The annual inpatient load is 14,670 for TAMC/DVA. REPLACE: The annual admission is 14,670 for TAMC/DVA. (2.)Section -1, 1.1. Scope of Work. Standard. ADD: The average daily sense 197. (3.) Section - 5, 5.13.2.1. Standard. DELETE: NOTE: Terminal (Discharge) Unit Cleaning is the responsibility of the gove rnment if required after hours. (4.) Section - 5, 5.13.6. TASK. DELETE: Entrances and entrance platforms are to be cleaned 3-days a week prior to the start of heavy traffic hours. REPLACE: Entrances and entrance platforms are to be cleaned 7-days a week prior to the start of heavy traffic hours. (5.) Section - 5, 5.14. 1 Standard. DELETE: The Contractor shall clean all interior surfaces of passenger and service elevators 3 days a week. (Elevators shall be cleaned at least once during each evening shift 3 times per week (M-W-F). REPLACE: The Contractor shall clean all interior surfaces of passenger and service elevators 7 days a week. (Elevators shall be cleaned at least once during each evening shift 7 times per week (Monday-Sunday). (6.)Page 67 of 173. DELETE: WAGE DETERMINATION AND FIRNGE BENEFITS: Each service emp loyee employed in the performance of this contract shall be paid the minimum monetary wage and fringe benefits required by Wage Determination No. 1994-2153 Rev (30), date of last revision 5/28/02 is available electronically at the following address: http:/ /www.ceals.usace.army.mil/netahtml/srvc.html or upon request to Karen Lee @ (808) 433-7008. REPLACE: Collective Bargaining Agreement (Pacific Environmental Services and Hawaii Teamsters and Allied Workers, Local 996 I B T) will be provided upon request vi a e-mail karen.lee@amedd.army.mil. (7.)QUESTION: Is seniority list currently available? If not, what is count of employees by years of service at Tripler AMC. ANSWER: Seniority list will be provided to the successor Contractor. Currently, Pacific Regiona l Contracting Office does not have seniority list nor count of employees by years of service.
