- This modification is issued to cancel the combined synopsis/solicitation. This is a combined synopsis/solicitation for commercial items or services prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation a written solicitation will not be issued. This solicitation is issued as a Request for Quote (RFQ) as follows. The solicitation clauses and provisions are those in effect through Federal Acquisition Circular 97-27. The Standard Industrial Classification (SIC) Code for these acquisitions is 5049 and the North American Classification System (NAICS) Code is 421490. The Food and Drug Administration (FDA) intends to issue purchase orders on a sole source basis to BioMerieux for the following equipment: Requisition 230097 (1) 2 each Vitek 120, W2102, Capacity for 120 cards, includes 10 card filling module/sealer unit, reader/incubator module, printer CC4 computer, bioLiaison, Terminal, and start up kit and installation. (2) 2 each Vitek Validation Package, W2405 includes CD Rom, templates for validation plan, installation operational and performance qualification and validation report. Also contains change control plan, all necessary attachments and on site technical assistance. (3) 40 each GPI test kit V1305. (4) 60 each GNI plus test kit V1316.shipping to FDA, 158-15 Liberty Avenue, Jamaica, NY 11433. Requisition 024420 (1) 1 each Vitek 32, W22100, capacity for 32 cards includes Command Module, Color Data Terminal, Multi Copy Printer, CC4 Computer, bioLiaison, Start up Kit and Installation. (2) 1 each Mini VIDAS 12 test capacity, W2111 includes keypad, screen, printer, reader and upload interface to LIS 2 sections with 6 positions each, start up kit, manuals, software, installation, training and 1 year warranty. Requisition A50917 (1) 1 each VITEK 32 Data Trac, capacity for 32 cards, includes Command Module, Color Data Terminal, Multi copy Printer, CC4 Computer, bioLiaison Start up Kit, Data Trac, Installation, and On site training is included. Shipping to FDA, 1521 West Pico Boulevard, Los Angeles, CA 90015. The following FAR Clauses shall apply to this Solicitation 52.212-1 Instructions to Offerors-Commercial, and 52.213-4 Contracts Terms and Conditions-Commercial Items. All responsible sources that can provide and meet the above requirements shall submit written quotations by the due date referenced above. The award is being issued on a sole source basis because the equipment is only available from the contractor with no known resellers.
