- Description
- Provide all labor, vehicles and materials, be fully licensed, insured, equipped and staffed to provide Ground Ambulance Services to transport scheduled (non-emergent) VA Medical Center, Minneapolis, MN and Twin Ports Outpatient Clinic (TPOPC), Superior, WI beneficiaries as directed; offeror must have a base of operations at which the physical plant houses ambulances, related equipment and personnel; provide appropriate personnel, vehicles, and equipment to provide a level of care so as to ensure that life-threatening situations and potentially serious injuries can be recognized for Basic Life, Advanced Life and Critical Care Advanced Life Ground Ambulance Services . Line Item quantities are based on historical data from the previous 12 months at time of issuance. Pricing will include incidental and miscellaneous costs associated with any transport at a Base Rate to include the first 10 loaded miles and a 15 minute wait time. Offers will be evaluated technically on facilities, vehicles, driver/driver qualifications, etc. Procurement is being made in accordance with FAR Parts 12 Commercial Item and 15 Negotiated Contracts. The solicitation will contain requirements for a 12-month base year with two (2) 12-month options to renew. Service will began October 1, 2002 through September 30, 2003 for Base Year. Proposal documents will be available on or about July 26, 2002 electronically with proposals due by close of business August 22, 2002. NAICS is 621910, $6 million in revenues.
