- ******THIS IS A REQUEST FOR PRICING INFORMATION*********** FISC Puget Sound is seeking pricing information on the following polyethylene bags: BLUE poly bags with label A) Bag, plastic, disposal, wet/dry asbestos waste, with label printed on one side of bag. B) 3 Sizes: 24"W x 48"L, 6-10 mil thick, 430 cases; 36"W x 48"L, 6-10 mil thick, 51 cases; 12"W x 24"L, 6-10 mil thick, 18 cases. C) Material: polyethylene, IAW CID A-A-3174, Type II, class 1, grade C, finish 2 D) Color: light blue, transparent E) Label message: "DANGER, CONTAINS ASBESTOS FIBERS, AVOID CREATING DUST, CANCER AND LUNG DISEASE HAZARD (shipyard address)" F) Label and format will be in accordance with OSHA hazard communications standard 20 CFR 1910.1200 (f) with the addition of the users address as follows: Puget Sound Naval Shipyard Bremerton, WA 98314 G) All letters, numbers and oval outline will be in white. H) The word "Danger" will have 2" high letters. All remaining letters and numbers will be ?" high. I) Package and ship/45 lb. container, 50 bags per box, ordered and shipped periodically as needed. GREEN poly bags A) Bag, plastic B) 3 Sizes: 36"W x 48"L, .008" thick, 530 cases; 24"W x 48"L, 008" thick, 520 cases; 14"W x 48"L, .008" thick, 310 cases. C) Material: polyethylene, IAW CID A-A-3174, Type II, class 1, grade C, finish 1 D) Color: green, similar to color #14097 per Fed. Std.# 595B E) Seam: Heat sealed seam 1/8" nominal width. Seam shall extend throughout entire width of bag. Seam to be ?" to ?" from the bottom of the bag. F) Package and ship/45 lb. Container, 50 bags per box, ordered and shipped periodically as needed. WHITE poly bags with label A) Bag, plastic, waste disposal, with label, gusseted 2 sides B) 2 Sizes: 24"W x 10"Deep x 36"High, 6 mil thick, 125 cases; 24"W x 10" Deep x 48"High, 6 mil thick, 460 cases. C) Material: polyethylene, IAW CID A-A-3174, Type II, class 1, grade C, finish 2 D) Color: white E) Label and letter size: "WASTE" in black 2" high block letters. F) Package and ship/45 1b. container, 50 bags per box, ordered and shipped periodically as needed. GREEN poly bags with label A) Bag, plastic, non-asbestos waste, disposal, with label B) 3 Sizes: 24"W x 48"L, .008" thick, 1400 cases; 36"W x 48"L, .008" thick, 300 cases; 14"L x 48"L, .008" thick, 300 cases. C) Material: polyethylene, IAW CID A-A-3174, Type II, class 1, grade C, finish 2 D) Color: green E) Label and letter size: "THIS BAG CONTAINS NO ASBESTOS MATERIAL", ?" high black letters, random print style, message printed twice per bag. F) Package and ship/45 lb. container, 50 bags per box, ordered and shipped periodically as needed. ***All bag quantities are estimated over a one year period.
