Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. A written solicitation will not be issued. This announcement constitutes the only solicitation. F9DOSA21980100 issued as a Request for Proposals. This is a full and open competitive requirement; the associated North American Industry Classification System (NAICS) is 711211and small business size standard is $6,000,000.00. A firm fixed price contract is contemplated. Award will be on or before 31 Aug 2002. Group Fitness Membership is needed for 30 people in support of the National Security Space Architect (NSSA). The offeror will provide fitness membership for 30 people and 30 membership transfers. The proposed award will be for one base year and two one-year option periods. BASE PERIOD: 01 Sep 2002 through 31 Aug 2003 0001 Group Fitness Membership 0002 Membership Transfer Fee OPTION PERIOD 1: 01 Sep 2003 through 31 Aug 2004 1001 Group Fitness Membership1002 Membership Transfer Fee OPTION PERIOD 2 : 01 Sep 2004 through 31 Aug 2005 2001 Group Fitness Membership 2002 Membership Transfer Fee Prospective offerors shall provide a unit price and extended amount for Group Fitness Membership CLIN 0001, 1001, and 2001 based on 12 months for each base period, option period one, and option period two. Prospective offerors shall provide a unit price and extended amount for Membership Transfer Fee CLIN 0002, 1002, and 2002 based on 30 members for each base period, option period one, and option period two. The Statement of Work, Instructions and Evaluation procedures are attached. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-08. You may contact Contract Specialist, Octavia Capers or Contracting Officer, Natalie Sitkowski by email for information regarding this solicitation. ============================================================================= Statement of Work for Physical Fitness Facility (Gym) Group Membership (As of 29 July 2002) 1. Introduction. The National Security Space Architect (NSSA) is a federal government organization located at 11242 Waples Mill Road, Fairfax, VA. The NSSA employees include active duty military personnel. The Department of Defense requires these personnel to be physically fit. 2. Program and Performance Objectives The NSSA requires a physical fitness facility (gym) for the use of its people, at no cost to those people. The NSSA will purchase a group membership covering 30 people at a Gym within a 1-mile radius of the NSSA Facility. This group membership will provide the same access to facilities and service that the Gym offers for sale to the general public. 3. Scope 3.1. Description of Services 3.1.1 The Gym will provide NSSA with a group membership covering 30 people at one time. The group members will have the same access to facilities and services that the Gym offers to the general public through membership such as locker rooms, towel service, and personal trainers (to be paid for by the individual) . To meet the minimum needs of DoD personnel for physical fit requirements, the Gym must have available to group members showers, dry saunas, steam rooms, swimming pool, whirlpool, aerobic exercise equipment, strength training equipment, flexibility training equipment, multi-purpose rooms, and classes. 3.1.2 After the initial assignment of 30 people to the NSSA?s group membership, the NSSA will pay the Gym a transfer fee for every old member replaced by a new member in accordance with the Purchase Order. The exception to the Gym policy is that the NSSA group membership is limited to total of 30 transfers per year. A single membership slot may have more than one transfer during the year, but the total number of transfer during the year for the NSSA group membership will not exceed 30. 3.1.3 The Gym will waive its normal 30-day cancellation notice for existing Gym members who will transfer to the NSSA group membership. Changing private monthly memberships to the NSSA group membership must occur at the beginning of the month, as the Gym will not prorate membership fees for periods less than a month. 3.1.4 When a new person is added to the NSSA group membership, the person will process into the Gym?s membership following the Gym?s normal procedure, except that the Gym will not require any credit card or bank account information. 3.2 Service Delivery Schedule 3.2.1. Service will start on 1 Sep 02. 3.3 Government Furnished Property and Services: None. 3.4 General Information 3.4.1 Deliverables: None. 3.4.2 Security: None. 3.4.3 Period of Performance: One full year with 2 options years 3.4.4 Place of Performance: The physical fitness facility 3.4.5 Hours of Operation: The normal hours of the physical fitness facility. 3.5 Appendices: None ========================================================================== ADDENDUM TO FAR 52.212-1 Instructions to Offerors?Commercial Items (Oct 2000) The Government contemplates award of a Firm Fixed Price Contract. PRICE: Offerors shall provide a unit price and extended amount for Group Fitness Membership CLIN 0001, 1001, and 2001 based on 12 months for each base period, option period one, and option period two. Offerors shall provide a unit price and extended amount for Membership Transfer Fee CLIN 0002, 1002, and 2002 based on 30 members for each base period, option period one, and option period two. TECHNICAL: Offeror shall provide a brochure or written information addressing paragraphs 2 and 3 of the Statement of Work. Contract award will be made to the lowest price acceptable offer, including options. =========================================================================== ADDENDUM TO FAR 52.212-2 Evaluation ?Commercial Items (Jan 1999) 1. BASIS FOR CONTRACT AWARD The evaluation of offers will be based on the standards outlined in the Statement of Work. Proposals will be acceptable or unacceptable based on an offeror?s ability to meet the requirements of the Statement of Work. The evaluation process shall consist of the following factors: (a) Price and price-related factors including options (b) Technical capability factors (Offerors ability to meet all the requirements of the SOW) 2. FACTORS (a) Price and price-related factors: Initially offers shall be ranked according to price, including options. (b) Technical capability factors: Next, the contracting officer will review offeror?s brochure or written information which addresses paragraphs 2 and 3 of the Statement of Work for acceptability.
