Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The North Amercial Industry Classification System (NAICS) Code applicable to this solicitation is 421450, Medical, Dental and Hospital Equipment and Supplies Wholesalers, and the small business size standard is 100 employees. This is a full and open competition (unrestricted). The VA Healthcare Network Upstate New York at Syracuse, NY intends to purchase Vela Ventilator Systems, brand name or equal. Mfg: VIASYS Healthcare. The components are eleven (11) Vela Ventilator with Standard Transport Stand, part number 15997; eleven (11) Support Arm, part number 15987; eleven (11) Internal Battery Upgrade Kit (Vela), part number 16048; eleven (11) Cable, RA, TBird, part number 15619; Vela Ventilator training program for one (1) biomedical technician to include tuition, travel, lodging, and per diem in accordance with GSA schedule. Two (2) sets of Users/Service manuals. Manufacturer: VISAYS Healthcare. Also, require allowance for trade-in - nine (9) Bear 3 and two (2) Bear 33 units. Salient characteristics of the equipment include: (1) Battery operation built into ventilator for 6 hours of operation without AC power, charges are part of system. (2) Graphic display to visualize the patient/ventilator interface including parameters such as work of breathing (WOB), Auto PEEP, and inspiratory/expiratory flow patterns. (3) Separate Exhalation Manifold and Flow Sensor units. (4) Medical air or internal compressor not needed to operate. (5) Run off of either high pressure oxygen or bleed-in low flow source. This solicitation document incorporates Federal Acquisition Circular 01-08 and VA Acquisition Regulation (VAAR) clauses as follows: 52.211-6 Brand Name or Equal; 52.212-1 Instructions to Offerors Commercial Items; 52.212-3 Offeror Representations and Certifications - Commercial Items; 52.212-4 Contract Terms and Conditions - Commercial Items; 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders Commercial Items with (b)(11), (12), (13), (14), (15), (16), (19)(iii), (21), (22) and (25) checked; and VAAR 852.270-4 Commercial Advertising. Offers to include completed copy of 52.212-3 Offeror Representations and Certifications - Commercial Items. Ths provision at 52.212-2, Evaluation of Commercial Items, applies to this acquisition. The evaluation criteria stated in paragraph (a) of this provision is as follows: (1) Technical capability of the item offered to meet the government requirement and (2) price. The system offered must meet all the salient characteristics specified above to be considered for award. Award will be made to the offeror proposing an acceptable system at the lowest overall price.
