Description
The Substance Abuse and Mental Health Services Administration (SAMHSA), Center for Mental Health Services (CMHS), through the Program Support Center, plans to award a firm-fixed price purchase order using simplified acquisition procedures. Professional Services Contract Entitled, ? Evaluation of Coalitions for Violence Prevention?. The purpose of this requirement is to develop and deliver an evaluation report to CMHS which identifies the level of success obtained at the end of the second year of the Coalitions for Violence Prevent program grant sites. The report will identify the levels of linkage and sustain ability achieved by grantees. Task 1. The Contractor shall submit a complete and detailed Work Plan designed to meet the purpose and objectives of this contract and to carry out its specific, delineated Tasks. Task 2. The Contractor shall develop a standardized protocol for performing the evaluation for GPO approval. This protocol will involve development of a survey based on benchmarks from pre-existing work and focus on fidelity to the key process benchmarks and key outcomes (e.g., sustain ability, collaboration, coordination and access.) Task 3 .a. The contractor shall review the required reports from grantees. b. The contractor shall implement the evaluation protocol including, but not exclusive to, the approved bench-marks and domains at 9 two year funded, and 9 three year funded grant-sites after GPO approval of the selected sites. Task 4. The contractor shall provide a separate analysis of grant sites which had 2 year awards and grant sites with 3 year awards. The contractor shall include issues and barriers for grantees. The contractor shall make recommendations for technical assistance to grant sites with 3 year awards. Task 5. The contractor shall provide an analysis of Coalitions for Violence Prevention grantee outcomes using GPO approved protocol and method. Task 6. The contractor shall identify evaluation needs of grant-sites. Task 7. The contractor shall provide a written report of progress at the end of each task or every 90 days whichever comes latest. The issue date is August 8, 2002; proposal due date is 5:00 PM local time on August 19, 2002. The period of performance for the award is twelve months after purchase order award. In accordance with FAR 5.102(a)(1), the solicitation will be made available on or about 10 day from the date of this publication. The internet address for downloading the solicitation is http://www.fedbizopps.gov. Any notification of changes to the solicitation (amendments) will be posted on the internet. Interested parties are responsible for monitoring the site to assure that they have the most up to date information regarding this acquisition. Anticipated award date September 30, 2002. The simplified acquisition threshold is $100,000.
