- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in sub-part 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (II) Solicitation F02601-02-T0131 is being issued as a request for quotation. (III) This solicitation incorporates clauses and provisions in effect through the Federal Acquisition Circular 2001-08. (IV) This requirement is being synopsized as a small business set-aside under North American industry classification system code 813110, standard industrial classification code 8661, size standard $6 million. SERVICE TO BE ACQUIRED: (V) Catholic Auxiliary Chaplain. Contractor will serve as the chaplain for the Catholic Parish at Davis-Monthan Air Force Base. Total units worked will not exceed 168 per year. A unit shall consist of a (complete worship service), or an individual counseling session. The Catholic Auxiliary Chaplains duties include conducting Catholic religious services (Mass on Saturday, Sunday, and Holy Days, and on other days as may be required), administer rites and sacraments( baptism, weddings, confessions, communion, last rites for the sick, funerals and memorials), and oversee the preparation of ecclesiastical documents to record such services. Provide emergency counseling regarding moral and religious matters. Contract start date is 1 October 2002 thru 30 September 2003. The Auxiliary Chaplain shall certify appropriate professional competence by providing evidence of ecclesiastical endorsement. If a substitute chaplain is required, he will be of equal competence of the contractor and acceptable to the Wing Chaplain. Notice of the substitution will be given to the Wing Chaplain or his designee 24-hours prior to the scheduled service or function. The government reserves the right to award to the ?other than low? offeror, if it is in the Government?s best interest. (VIII) Provision 52.212-1 Instructions to Offerors- Commercial Items applies. (X) Offerors are required to submit a completed copy of 52.212-3 Offerors Commercial Items with its offer. (XI) Clause Representations and Certifications- 52.212-4 Contract Terms and Conditions- Commercial Items and any addenda the clause applies to this acquisition. (XII) Clause 52.212-5 Contract Terms and Conditions Require to Implement Statutes or Executive Orders-Commercial Items applies to the award document. (XVI) Quotations are due to the 355th Contracting Squadron, 3180 S. Craycroft Rd. Davis-Monthan AFB, AZ 85707, by 15 August 2002, 1500 local time. Arrange this with the POCs listed in XVII.
