Description
(i) This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) Solicitation of Purchase Request F73SVS21300600 is issued as a request for quotation (RFQ). (iii) This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2001-08. (iv) This acquisition is for full and open competition. The associated NAICS code is 713940 and small business size standard is $5,000,000.00. (v & vi) ) Line item 0001, Provide instructors to teach aerobics, spinning, yoga, and water aerobics classes as detailed in the statement of work. Qty. 1455 (vii) PROJECT: 1. SCOPE OF WORK: Contractor shall conduct and administer all aspects of the exercise portion of the Weight Management and Fitness Enhancement Programs held in the Base Fitness Center at Elmendorf Air Force Base, Building 9510. 2. PERSONNEL: The contractor or designated representative shall meet with government personnel designated by the Contracting office to discuss immediate problems. The Contractor or representative shall respond within four hours after notification. The contractor shall provide the telephone number of the person(s) to call should the need arise. 3. EMPLOYEE IDENTIFICATION: Contractor personnel must be recognizable as such while in the Fitness Center. This should be accomplished by wearing distinctive athletic attire. The contractor is responsible for acquiring and maintaining an appropriate number of uniforms to meet their need at their expense. 4. SPECIFIC DUTIES: Position is to manage, supervise, and conduct the Elmendorf Air Force Base Weight Management and Fitness Enhancement Program. Contractor will be responsible for the following: a. Individual must provide to the Contracting Office personnel documents certifying accreditation in physical conditioning, weight management, or certification from other recognizable institutes. As a minimum the individual must be certified by a nationally accredited organization as an aerobic instructor (i.e. AFFA, ACE, Dante). Classes will consist of an aerobic type workout. Classes should be well-rounded and include, but not limited to: step aerobics, slide aerobics, high impact, low impact, modified, water aerobics and pre-natal instruction. NOTE: At least one class taught during the day must have a person trained to give advice and instruction in pre-natal programs. The contractor will submit a monthly schedule to the Athletic Director for approval. The schedule will include the instructors name, type of class, and time. b. Must keep current and provide proof of certification in Cardiopulmonary Resuscitation as outlined by the American Medical Association or the American Red Cross. c. Contractor will perform classes using the following schedule: Aerobics: Oct - Mar classes at 0600, 1130, 1715, and 1830 hrs Monday - Friday, Saturday at 1130. Apr  Sep classes at 0600, 1130, and 1730 hrs Monday - Friday, Saturday at 1130. Spinning: Mon, Wed, Fri at 1100 and 1730, Sat 1100. Water Aerobics: Mon, Wed, & Fri at 1800. Yoga: Monday 0930, 1130 & 1830; Wed 1130 & 1830; Fri 0930. These times are subject to change based on the needs of the active units. Contractors will be notified 30 days in advance of any changes in class times. d. Contractor shall provide all tapes and music requirements for the classes. e. Contractor will ensure that all equipment (steps, mats, bands, music equipment etc.) is used according to the Fitness Center?s policies and are properly put away by the students or instructor after class. f. The contractor will handout at least 10 customer comment cards per day as provided by the fitness center. g. The contractor shall be financially responsible for any damage caused to fitness center equipment while it is under their control ( i.e. stereo system, microphone/headset, etc.) to include repair/replace, maintenance, shipping and handling of damaged equipment. h. The contractor will record an accurate headcount for each class performed using forms provided by the fitness center. i. The contractor will perform a check of student?s heart rate at least three times per class. j. The contractor will limit the number of classes taught by one instructor to no more than two classes per day. 5. The following holidays will be recognized and no instruction will be necessary: Thanksgiving Day and Christmas Day. For other Federal Holidays and Down/Family Days only one aerobic class, 1130, and one spinning class will be taught. Classes will be taught on Good Friday. Delivery terms shall be FOB Destination, Elmendorf AFB, Alaska 99506. (viii) FAR clause 52.212-1, Instructions to Offerors--Commercial, applies to this acquisition. (ix) FAR clause 52.212-2, Evaluation--Commercial Items, is not applicable. (x) Offerors are advised to include a completed copy of the provision at FAR clause 52.212-3, Offeror Representations and Certifications-Commercial Items, with the quote. Offerors must also include a completed copy of the provision at DFARS 252.212-7000, Offeror Representation. (xi) FAR Part 52.212-4 Contract Terms and Conditions-Commercial Items applies to this acquisition. (xii) FAR clause 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders--Commercial Items, applies to this acquisition. For paragraph (c) the following clauses apply: 52.222-41 Service Contract Act of 1965, As amended, 52.222-42 Statement of Equivalent Rates for Federal Hires, and 52.222-47 SCA Minimum Wages and Fringe Benefits Applicable to Successor Contract Pursuant to Predecessor Contractor Collective Bargaining Agreement (CBA). FAR 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Order - Commercial Items (Apr 2001) (DEVIATION). DFAR clause 252.212-7001, Contract Terms and Conditions Required to Implement Statutes- Commercial, also applies to this acquisition. Only the following FAR clauses cited in the clause are applicable. 52.203-3, Gratuities, 252.247-7023, Transportation by Sea and Air, (xiii, xiv, & xv) N/A.I DFARS 252.222-7000 Restrictions on Employment of Personnel (Mar 2000)(IAW DFARS 222.7003). Wage Determination No. 94-2017, Revision 27 dated 5/28/2002, hereby applies. This is a DO rated order with priority identification symbol of S10. A FirmFixed Price purchase order will be issued under the Simplified Acquisitions Procedures. (xvi) Submit signed and dated offer to 3CONS/LGCB at or before 2 September 2002 4:OO PM Alaska Standard Time. Quotation shall be signed, dated and submitted on your company?s letterhead or quotation forms. Quotations may be submitted via fax. Contractor must provide a copy of accreditations, as required by item 4 (a) of this notice, with the quotation. (xvii) All questions concerning this amendment must be submitted in writing and faxed to SSgt Anthony J. Smith at (907) 552-7496. No telephonic responses will be processed.
