Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This acquisition will be executed in accordance with FAR Part 13.5 - Test Program for Certain Commercial Items utilizing Simplified Acquisition Procedures. The Social Security Administration (SSA) Headquarters, 6401 Security Boulevard, Baltimore, MD 21235 has a requirement for a contractor to provide quarterly planned maintenance services on all electrical (Lead Acid Battery) and gasoline (Internal Combustion) powered material handling equipment. The equipment is located at various warehouses and loading docks in Baltimore, Maryland. The contractor shall perform scheduled planned maintenance on all equipment to include, but not limited to the following: Electrical (Lead Acid Battery) Powered Equipment: Structural/Visual Inspection - oil leaks, tires and wheels, caster assemblies; Operational Test - brakes, speed controls, wire guidance; (3) Battery and Cables - acid levels, corrosion, connector condition; (4) Cleaning and Lubrication - air clean truck, lubricate miscellaneous linkage, lubricate all fittings; (5) Brake System - check fluid levels/leakage, shoe and pad condition, master/slave cylinders; (6) Drive Axles Units - fluid levels, wheel mount bolts, unusual noises; (7) Reach and Tilt Cylinders - mounting, cylinder condition (leaks, nicks, etc.), adjustments; (8) Mast Upright & Carriage - operation, carriage condition, wear and/or damage; (9) Hydraulic System - oil levels and condition, pumps (mounts and seals), couplings; (10) Steering System - freeplay, power steering operation, cylinder condition; (11) Electrical Test - power cables, wiring, brush and commutator. Gasoline (Internal Combustion) Powered Equipment: (1) Test Drive Truck - drive train noise, steering operations, service brake operation, inching operations, transmission operations, clutch operations, hydraulic system operations, engine performance, parking brake operation, pedal pads and linkages, return to neutral; (2) Cleaning & Lubrication - air clean truck/radiator, lubricate truck; (3) Engine - check for leakage, check oil level, replace oil and filter, check mounting; (4) Cooling System - coolant level/condition/leaks, degree protection, inspect fan and fan control, inspect water pump; (5) Fuel System - check accelerator and return spring, check choke operation, check for fuel leakage, check solenoid, clean/replace filter; (6) Air Intake & Exhaust - clean or replace air filter, replace damaged hoses or clamps, check muffler & exhaust system; (7) Clutch - throw out bearing/linkage, pedal adjustment as needed; (8) Transmission - check fluid, levels/leakage, check condition of fluid, replace filter as required; (9) Ignition & Cracking Systems - check neutral start, check distributor condition, check writing condition; (10) Battery & Cables - clean and check battery terminals, check battery fluid levels; (11) Charging System - alternator mounting; (12) Gauges/Lights/Indicators - hour meter operation, gauges (all operational), lights (all operational), horn; (13) Axles & Wheels - differential level, check wheel bearing play, check lug condition, check tire condition, check air-pressure in tires; (14) Brake System - check for leakage, check cylinder fluid level, check pedal for free travel, check pedal drift; (15) Steering System - check system for leakage, check tilt column operation; (16) Hydraulic System (Where Applicable) - check for leakage, check condition of fluid and level, clean/replace breather, replace filter as required, check hose condition, check cylinder condition and mounting. The contractor is required to prepare on a quarterly basis two written reports. A report shall be prepared on each piece of material handling equipment serviced to include a checklist annotating the planned maintenance completed on the material handling equipment fleet. A second report shall be prepared for recommended repairs. This should be prepared after each piece of equipment has been serviced and should contain recommendations on repairs needed to each piece of equipment to bring it in compliance with the manufacturers specifications. This report should be dated with the actual date that the planned maintenance service was performed. Note: Submittal of second report for recommended repairs does not guarantee award of the repair service. All reports should contain the following: Manufacturer's Name, Model No. of Unit, Serial No. of Unit, SID No. of Unit and Actual PM date. All interested contractors shall contact the Contracting Officer below for a list of the equipment Manufacturer, Model No., Serial No. and SID No. for all material handling equipment for which service is required for this acquisition. The contractor shall be in compliance with the requirements of the Occupational Safety and Health Administration (OSHA) regulation, Title 29, 1910.147, the Control of Hazardous Energy (Lockout/Tagout), including but not limited to, employee training on lockout/tagout procedures and ensuring that employees comply with the procedures therein. The Government contemplates awarding a Firm Fixed Price contract for this effort. The solicitation number for this requirement is SSA-RFP-03-0504 and Request for Proposal (RFP) is requested. The solicitation document and incorporated provisions and clauses are those in effect through FAC-01-08. This requirement is being set-aside for 100% Small Business concerns. Offers received from other than small business concerns will be deemed unacceptable and will not be considered. The North American Industry Classification System (NAICS) code for this requirement is 811310 and the standard size is $6 million dollars. The period of performance is for one (1) twelve (12) month Base Year with four (4) twelve (12) month Option Years. Offers are required to submit a price for the Base Year and for each of the four (4) Option Years. The following Federal Acquisition Regulation (FAR) clauses and provisions are incorporated by reference and apply to this acquisition: (NOTE: Full text of all clauses and provisions can be accessed via the internet at http://www.arnet.gov). The provision at FAR 52.212-1 - Instructions to Offerors - Commercial Items (OCT 2002) applies to this acquisition. The following is the addenda to FAR 52.212-1 applies; (b) Submission of offers is supplemented to read, "Offers shall submit four (4) copies of their Business and Price Proposal. Offerors shall submit three (3) citations/references. Citations/references must have been completed within the last three (3) years or be currently active"; (c) is changed to read, "90 calendar days"; (f)(2)(i) is changed to read, "Any offer, modification, revision or withdrawal of an offer received at the address specified for receipt of offers after the exact date and time specified for receipt of offers will not be considered for award under any circumstances"; (d), (e), (h) and (i) are not applicable. FAR 52.212-2 - Evaluation - Commercial Items (JAN 1999) applies to this acquisition. The following addenda to FAR 52.212-2 applies; (a) is supplemented to read, "The following factors shall be used to evaluate offers: Experience, Past Performance and Price. The Government will make comparisons among offerors who submit acceptable offers until the offeror who represents the best overall value has been determined. Award will be made on a best overall value/benefit basis taking into account the above factors". Offerors must include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications - Commercial Items (JUL 2002). FAR 52.212-4 - Contract Terms and Conditions - Commercial Items (FEB 2002) applies to this acquisition. FAR 52.212-5 - Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items (MAY 2002) applies to this acquisition.
