Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation is issued as a Request for Quote (RFQ) under the number FA4452-02-Q-A289. This document and incorporated provisions and clauses are those in affect through Federal Acquisition Circular 2001-08. In accordance with FAR Subpart 19.2, it is the policy of the Government to provide maximum practicable opportunities in its acquisitions for small business concerns. In accordance with FAR Subpart 19.5, this acquisition has been exclusively set aside for participation by small business concerns. The North American Industry Classification System (NAICS) code for this acquisition is 611430. Small business size standard is $6.0M. The AMC Contracting Flight intends to issue a sole source purchase order under the test program described in FAR 13.5 to LivingWorks Education Inc., #208, 1615 10th Ave., S.W. Calgary T3C0J7 Canada, for the following: The contractor shall provide 1080 each -- Applied Suicide Intervention Skills Training (ASIST) Prepaid Kit: Day 1, Day 2, Workbook/Handbook, Certificate and Support Fees, including shipping and handling. Brand Name or Equal.
