Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 of the FAR, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number, F45613-02-Q-B042, is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-07. The North American Industry Classification System (NAICS) Code is 811219, size standard $6.0 Million. This project is 100% Set-Aside for Small Business Concerns. The contractor shall provide all labor, tools, facilities supervision, and transportation necessary to provide preventative maintenance and repairs on government-owned equipment for the 92nd Medical Group, Fairchild AFB, WA in accordance with the attached statement of need (SON). The following provisions and clauses can be viewed in full text at http://farsite.hill.af.mil/. The provisions at FAR 52.212-1, Instructions to Offerors-Commercial Items (Oct 2000), and FAR 52.212-3, Offeror Representations and Certifications-Commercial Items (Jul 2002) apply to this acquisition (a completed-signed copy of this provision shall be submitted with any quotation and can be printed from the aforementioned website. The clauses at FAR 52.212-4, Contract Terms and Conditions (Feb 2002), FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items (May 2002) and DFARS 252.204-7004 (Nov 2001) apply to this acquisition. The following clauses are incorporated under paragraph (b) of FAR 52.212-5; FAR 52.222-21, Prohibition of Segregated Facilities; FAR 52.222-26, Equal Opportunity, FAR 52.222-35, Affirmative Action for Disabled Veterans and Veterans of the Vietnam Era; FAR 52.222-36, Affirmative Action for Workers with Disabilities; FAR 52.222-37, Employment Reports on Disabled Veterans and Veterans of the Vietnam Era; FAR 52.225-1, Buy American Act-Balance of payments Programs-Supplies; FAR 52.222-19, Child Labor-Cooperation with Authorities and Remedies, FAR 52.225-3, Buy American Act-North American Free Trade Agreement-Israeli Trade Act-Balance of Payments Program, Alt I, FAR 52.225-13, Restrictions on Certain Foreign Purchases; and, FAR 52.232-33, Payment by Electronic Funds Transfer. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. Quotations are due 3 September 02 by 4:00 p.m. Pacific Daylight Savings Time. Faxed or E-mailed proposals are allowed and will be considered. Submit proposals in accordance with FAR 52.212-1 and addressed to TSgt Eduardo V. Alfaro at the address indicated in this announcement and shall include the following items: 1) Pricing schedule, 2) Completed FAR 52.212-3 Representations and Certifications, with Alternates I and II (accessible at the Air Force FAR site http://farsite.hill.af.mil), and 3) list of your customers having similar requirement (provide name, telephone number, and point of contact).
