NA THIS IS COMBINED SYNOPSIS/SOLICITATION FOR COMMERCIAL ITEMS PREPARED IN ACCORDANCE WITH THE FORMAT IN FEDERAL ACQUISITION REGULATION (FAR) SUBPART 12.6, AS SUPPLEMENTED WITH ADDITIONAL INFORMATION INCLUDED IN THIS NOTICE. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION; QUOTATIONS ARE BEING REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED. *****This solicitation, DABT19-02-Q-0025, is issued as a Request for Quotation (RFQ), 100% Set-Aside for Small Business. The solicitation document and corpor ate provisions and clauses are those in effect through Federal Acquisition Circular 2001-08 and DFARS Change Notice 20020531. The associated NAICS code is 314999 and small business set-aside size standard applicable to this procurement IS 500 EMPLOYEES. This acquisition is a Total Small Business Set-Aside. *****The Directorate of Contracting (DOC), Ft. Leavenworth, KS has a requirement for the removal and installation of Carpet. Carpet will be installed between the hours of 5 Pm and 1 AM central standard time. Furniture will remain in the room during the installation of the carpet. A Site Visit is scheduled for 4 Sep 02 at 9:00 AM (CST) and interested parties must email a request to attend prior to 1:00 P.M., 3 Sep 02 to sharshef@leavenworth.army.mil . Details in regards to the site visit will be forwarded upon receipt of this request. Offerors shall submit quotes based on scope of work given at site visit. The Contracting Officer will evaluate on the basis of information furnished by the officer or id entified in the offer and reasonably available to the Contracting Officer in accordance with FAR 52.214-21 Descriptive Literature. Warranties other than those specifically stated should be, at a minimum, the industry standard commercial warranty offered. The Government anticipates award of a firm fixed price contract, with installation to begin in October depending on arrival of the carpets. Delivery and acceptance is at the National Simulation Center, 410 Kearny Ave, Ft. Leavenworth, KS 66027. The offe rors should submit with their quotation: (1) Cost proposal which shows each individual service to be performed s, proposed unit cost, total cost per item, and total price for all items; (2) Supporting information for proposed functional, or performance c haracteristics specified above; (3) Completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications ? Commercial Items (MAY 2002), and Alternate I (APR 2002); (4) Contractor?s Duns and Bradstreet (DUNS) number; (5) Contractor?s Cage Code; and (6) An affirmation that it has an active registration on the Central Contractor Registration (CCR), which is at the website www.ccr2000.gov.
