- NA This is a combined synopsis/solicitation for commercial items prepared IAW the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being reque sted and a written solicitation will not be issued. Solicitation Number DADA13-02-T-0078 applies and is issued as a Request for Quotation (RFQ). Solicitation Line Items are as follows: Line Item 0001, 2 EA, Nidek Confoscan 3 System, or equal, and shall consist of the following components and salient characteristics, as a minimum. Fully Digital Ophthalmic Microscope; Integrated Lift Table; Personal Computer, Pentium III 850 Mhz; Windows 2000; 30 Gb Hdd; 256 Mb RAM, 40X CD RW; d. Phillips 151AX Flat Sc reen ; NAVIS ver 3.4.0 Cell Analysis Software, Zeiss 40x immersion lens. The system shall be suitable for in vivo examination of the cornea at a cellular level and possess the following features and capabilities, as a minimum: Gel immersion coupling tec hnology to allow coupling of microscope to cornea without the need for applanating the cornea; Software assisted alignment and recording; Objective evaluation and graphical display of the light intensity reflected by each corneal layer; Optical pachymetry of the cornea, as well as the ability to assess pachymetry of internal structures, such as flap thickness after LASIK; Digital recording of exam; Manual and automatic cell counting and cell density calculation; Statistical analysis of polymegathism and pl eomorphism. Line Item 0002 Manuals-Two (2) copies each of the following: 1) Operating Instructions; 2) Maintenance Instructions, and 3) Schematic Drawings. If it is standard practice to provide copies of these manuals at NO cost please indicate by input ting N/C or $00.00 where you would otherwise insert the price.Warranty-Equipment shall include a warranty of one year or commercial practice. The warranty shall become effective at the time of installation. Warranty details, to include the effective date shall be submitted with the quotation; Warranty-Software shall include a warranty of one year or commercial practice. The warranty shall become effective at the time of installation. Warranty details, to include the effective date shall be submitted w ith the quotation. Software updates shall be provided, at no additional cost to the Government as they are released and commercially available. Installation shall occur at a mutually agreed upon date and time following Government acceptance. The contractor shall provide on-s ite operators training at a mutually agreed upon date and time, but not longer than 30 days after installation of the equipment. Responses shall include proposed delivery date from date notified of award. Prices shall include all taxes and freight charges . Request FOB destination pricing. Delivery will be made to Madigan Army Medical Center, Tacoma WA. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-08. This solicitation is issued as full and open competition with a SIC Code of 3827 and size standard of 500 employees. The following clauses apply to this solicitation: FAR 52.212-1, Instructions to Offerors?Commercial Items; however, sub-paragraphs (g) and (h) are deleted as this is an RFQ. FAR 52.212-2, Evaluations, Commercial Items with the following factors included in FAR 52.212-2(a) used to evaluate offers: Technical Capability, Past Performance, and Price. The quote shall include technical information consisting of, descriptive literature for the system being quoted, to include all accessories; a minimum of 2 references with whom past performance can be verified to include points of contact, telephone numbers and a total contract price. FAR 52.211-6, Brand Name or E qual applies to this solicitation. Respondents shall submit, as a part of their quote, completed copies of the provisions at FAR 52.212-3 w ith Alt I, Offeror Representations and Certifications?Commercial Items and DFARS 252.212-7000, Offeror Representations and Certifications?Commercial Items. FAR 52.212-4, Contract Terms and Conditions?Commercial Items, applies to this acquisition; addenda are as stated herein. FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders?Commercial Items applies to this acquisition. Once the Contracting Officer has decided with whom a contract will be entered, a bilateral contract requiring the quoter?s signature will be issued. A contract will be in existence once both the quoter and the Contracting Officer have sig ned the document. Wherever the words offer, proposal, offerors, or similar terms are used in this solicitation; they shall be read to mean quote, quotation, or quoter or similar corresponding terms to reflect that this solicitation is a Request for Quotat ion. Current registration in the Central Contractor Registration Data Base is required in order to receive an award. Quotes may be submitted via e-mail; facsimile to 253-968-4922; US Postal Service or by some form of Express Mail. Quoters are advised that the US Postal Service Express Mail is delivered to an area within Madigan Hospital, which is physically separate from the contracting office. This method of delivery could result in your quote being received late. All submissions should include the RFQ number, the company name and a company point of contact.
