- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number F41614-02-T-0065 is hereby issued as a Request for Quotation. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-08 Notice: This acquisition is set aside for small business and is under NAICS code 621999, not to exceed $8.5 mil. Requirement is for Contractor to provide nutrition education for the entire fiscal year 2003. The service description specification will be in accordance with the attached Statement of Work. 0100: Weight Management Classes 4 per month 48 each, 0101: Diabetes Classes 1 class per month 12 each, 0102: Hyperlipidemia Classes 1 class per month 12 each, 0103: Private Consultation Sessions 80 per month 960 each, 0104: Commissary tour 1 every six months 2 each, 0105: Dining Hall tour 1 per quarter 4 each, 0106: Cooking Class 1 class per quarter not to exceed 2 hours per class 8 each, 0107: In-Service PCO training 1 per quarter 4 each, 0108: Dietary Supplement Training 4 per month 48 each, 0109: Nutritional Specialty Education 1 hour class per month 12 each. 0110: Documentation hours NTE 50 hours total 50 each, 0111: Class and consult preparation NTE 48 hours total 48 each. The following clauses and provisions are incorporated and are to remain in full force in any resultant purchase order: FAR Clause 52.212-1, Instructions to Offerors - Commercial Items, (OCT 2000) is incorporated by reference. It is amended to read: Submit signed and dated offer to 17 CONS/LGCB, 210 Scherz Blvd., Goodfellow AFB, TX 76908 at or before 1630 (4:30 P.M.) CST, 20 Aug 02. Submit signed and dated quotations on company letterhead or quotation form. Quotations may be faxed to (915) 654-4427. The government reserves the right to make award on the initial quote without discussions. Evaluation will be based on price and technical capability. Include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications Commercial Items (MAY 2002),with quote. An electronic version is available for download along with a quote sheet. FAR clause 52.212-4 (FEB 2002), Contract Terms and Conditions-Commercial Items, is incorporated by reference and applies to this acquisition. FAR clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items (MAY 2002) applies to this acquisition and is incorporated by reference.
