- Description
- The Federal Bureau of Prisons, Mid-Atlantic Regional Office, Lexington, Kentucky, intends to issue a solicitation, RFQ 10907-010, for Opthalmology services for the Federal Medical Center in Lexington, Kentucky. The contract type is a firm-fixed price, requirements type contract. The contract period shall consist of a 12 month base period and four 12-month option periods for renewal. The estimated quantities for the service is 12 four hour sessions per year, 10 office visits per year and 15 surgical procedures per year. Services include but are not limited to: providing onsite ophthalmology service to include eye exams, fundiscopic exams, diagnosis, treatment and prescribing medication as allowed by law, outpatient diagnostic procedures to include tonometry, indirect and direct fundiscopy, visual fields, and an other special clinical procedures approved by the Clinical Director. Requirements include: must be a graduate of an AMA accredited school of medicine and must have completed an AMA approved internship/residency program and possess current speciality board certification and state licensure without restrictions, to practice medicine. The solicitation will be made available on or about May 8, 2002, and will be distributed solely through the General Services Administration's Federal Business Opportunities website http://fedbizopps.gov/. Hard copies of the solicitation will not be available. The site provides downloading instructions. All future information about this solicitation, including solicitation amendments, will also be distributed solely through this site. Interested parties are responsible for monitoring this site to ensure that they have the most up to date information about this acquisition. The anticipated date for receipt of quotations is June 9, 2002. All responsible sources may submit a proposal which will be considered.
