- This is a combined synopsis/solicitation for commercial items prepare= d in accordance with the Federal Acquisition Regulation (FAR) sub part 12.6, as supplemented with additional information included in this notice and incorporates provisions and clauses in effect through FAC 2001-09. The solicitation number for this Request for Quotation is 516-077-02. This BOS announcement constitutes the only solicitation issued. A separate written solicitation document will not be issued.=20 This solicitation is 100% set aside for Small Business. The North American Industry Classification Code is 421450 and the small business size standard is considered 100 employees. Numbered Note no. 1 applies to this solicitation. The VA Medical Center, Bay Pines, FL, is seeking a contractor to provide a brand name or equal for the following equipment: One vascular diagnostic system, Parks Flo-Lab Model 2100.=20 The system contains the following salient characteristics: Pentium III 1.5 GHz, 128 MB Ram,19 GB hard drive, 56 K Modem, 1.44 floppy drive, 36X CD-ROM, Zip Drive; Windows 98 Operating system; 17=94 flat panel color monitor, Hewlett Packard 900C series printer; Software with special features as used on Parks Flo-Lab 2100 VIP; two frequency, bi-directional Dopplers (4&8 MHz); forward and reverse Doppler Flow indicators; dual channel PPG; dual channel PVR; automatic cuff inflator with linear and programmable air pump to allow even inflation and deflation of pressure cuffs. Pump can also be programmable by user for inflation and deflation rate; twelve button remote control: Doppler volume and mute, navigation arrows, auto scale, and data storage; single/dual strip chart recorder; mobile cabinet with storage space; complete cuff package: 2 CC17, 2 SC12ex, 6 SC12, 4 SC10, 2 TMC7, 2 SC5, 2 DC2.5, 2 DC1.9, 2 PC2.5, 1 PC1.9; Multi-port Cuff selector; accreditation software with all software upgrades free for life of the unit; computer is fully net-workable and interfaces with the PAC system; system is not computer dependent, that is, all exams can still be performed if the computer should fail; manual analog input to override computer preference; Doppler spectrum analysis; multi-media capability; system includes built-in protection using a =93medical grade=94 (UL 544 spec) isolation transformer for added patient and technician safety; Doppler probes can be =93cold gas sterilized=94 and used in a sterile field; includes a one full day on site training for which trainees receive 8 CME credits from SVT; warranty is two years parts and labor. Delivery terms shall be FOB origin to the VA Medical Center, Warehouse, Bldg 100, 10000 Bay Pines Blvd, St. Petersburg, FL 33708.=20 Proposals shall be submitted on an =93all or none basis.=94 Partial proposals shall be rejected. If submitting on a brand name equal basis, sufficient technical data must be included for the government=92s evaluation. This solicitation is issued subject to the availability of FY 2002 appropriations. Offerors responding to this announcement shall provide all information contained in the Federal Acquisition Regulation (FAR) Clause 52.212-1, Instructions to Offeror=97Commercial Items.=20 Offerors shall include a completed copy of provision FAR 52.212-3, Offeror Representations and Certification=97Commercial Items and VAAR 852.219-70 Veteran-Owned Business.
