Description
This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. The solicitation number RFQ-030-4384-02 is issued as a request for quotations and incorporates provisions and clauses that are in effect through FAC 01-07. Procurement will be on an unrestricted basis. The NAICS code applicable to this acquisition is 562219. For a company to qualify as a small business, the small business size standard is $10,500,000 annual gross income for the last 3 years. USDA, ARS has a requirement for the removal and disposal of a research irradiator. The irradiator is a U.S. Nuclear, Model GR-12, Serial Number 911. It was originally purchased in 1968 and the initial assay of the Cobalt-60 activity was 4,500 Curies as of December, 1968. Estimated current activity is 55 Curies as of June, 2002. Contractor must provide the following materials and services: 1. Furnish all labor and materials necessary to properly package and prepare the irradiator and its radioactive sources for shipment. Ship the irradiator and its sources to a properly licensed facility for either beneficial reuse, source recovery or disposal. 3. Prepare all shipping documentation for shipment of the source for either processing or disposal. All freight charges will be paid by contractor. 5. Provide a Certificate of Possession or Transfer for the source upon shipment. Prior to receiving award, the contractor must provide a valid radioactive materials license and other necessary certificates for the transport and possession of the device. USDA will furnish forklift required for executing this project at site. USDA will provide a current leak test certificate for the device. The government anticipates award of a contract resulting from this solicitation to the responsible offeror whose offer conforming to the statement of work will be most advantageous to the Government, price and other factors considered. Service Contract wage rates will apply to this procurement. The following factors shall be used to evaluate offers and are approximately equal in order of importance. 1. Price. 2. Experience: offer to be considered shall list at least 3 projects of a similar nature with the past 3 years. 3. Past Performance: offer to be considered shall include 3 reference that can attest to offeror?s past performance; including contact name and telephone number. PAYMENT: Any award made under this solicitation is subject to 31 CFR Part 208 which requires all payments made by the government to be made by electronic funds transfer. Vendors may meet this requirement by acceptance of government purchase card or providing a Vendor Express Number. If a company has not previously enrolled in the Vendor Express Program with USDA, they should call the National Finance Center at 1-800-421-0323 for an enrollment package. All offers must include with their quotation a completed copy of FAR 52.212-3. The following FAR clauses apply: Data Universal Numbering System (DUNS) Number, 52.204-6; Instructions to Offerors-Commercial Items, 52.212-1; Evaluation of Commercial Items awarded to a responsive/responsible offeror at a fair and reasonable price, 52.212-2; Offeror Representations and Certifications-Commercial Items, 52-212-3; Contract Terms and Conditions-Commercial Items, 52.212-4. The following FAR clauses cited in 52.212-5 are applicable to this acquisition: 52.203-6, 52.222-35, 52.222-36, 52.222-37, 52.225-3, 52.225-13, 52.225-9, 52.225-18, 52.225-21, 52.247-64. All interested/qualified sources who can provide the service stated above must respond by 4:30 PM EDT, August 14, 2002.
