Description
The Defense Finance and Accounting Service has a request for Membership to full service physical fitness facilities for civilian and military personnel assigned and working in the Emmett J. Bean Federal Center, Indianapolis, IN. This is intended to award on sole source basis to YMCA of Greater Indianapolis, Indianapolis, IN. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; a written solicitation will not be issued. The solicitation number is IQI3P1P61022AA, which is a Request for Quotation (RFQ). All responses from responsible sources will be considered. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-27. The NAICS code is 713940. Line Items as follows: 0001. Memberships for 194 Active Duty Military and EREC, National Guard, OCAR Pay Support and U.S. Army Finance Command and Veterinary Services, and DCMA for the period October 1, 2002 through September 30, 2003 in accordance with statement of work; 0002. 225 Corporate Memberships for DFAS-IN Civilian Personnel for the period October 1, 2002 through September 30, 2003 in accordance with statement of work (Cost per employee shall be based on the following percentages: 150 @ 30% and 75 @ 15%). 0003. Option Year 1, services as described in Line Item 0001 for the period October 1, 2003 through September 30, 2004. 0004. Option Year 1, services as described in Line Item 0002 for the period October 1, 2003 through September 30, 2004. 0005. Option Year 2 services described in Line Item 0001 for the period October 1, 2004 through September 30, 2005. 0006. Option Year 2, services as described in Line Item 0002 for the period October 1, 2004 through September 30, 2005. Statement of Work 1) Access to fitness facilities, qualified instructors, formal health programs and services; 2) individual membership cards to be issued by the fitness facility personnel; 3) Privileges of regular membership to included unlimited access at any of the contractors facility locations; 4) Services to include, but not limited to, use of an indoor gymnasium, community center or recreation center complex with indoor swimming pool, cardiovascular equipment, stationary and free weights, workout room, shower and secured locker facilities. Facility to be available for use at a minimum from 5:30am until 9:00pm Monday through Friday; 5) Health, wellness and educational programs, to include personal fitness evaluations, weight management, and smoking cessation. Wellness programs to be conducted by instructors with current certification from either American College of Sports Medicine, American Council on Exercise, YMCA or National Commission for Health Education; 6) Primary facility is within reasonable walking distance (within 15 minutes) of the Emmett J. Bean Federal Center, 8899 East 56th Street, Indianapolis, IN 46249; 7) Validate that the civilian employees are enrolled in the DFAS Wellness program prior to issuing a fitness club corporate membership card; 8) Provide listing of all cards issued each month and a monthly utilization report of all membership cards. The listings must be provided NLT the tenth working day of the following month to the COR; 9) Provide monthly billing to the COR for the agency portion of the military and civilian corporate memberships; 10) Direct billing to the civilian employee for their balance of the corporate membership costs; 11) The Government retains the right to discount their portion of the employee membership at 15% and 30% and to terminate the membership when utilization drops below an average of two visits per week over a period of two months. Applicable Regulations and Documents: 1) DOD Directive 1010.10; 2) Health promotion and the American College of Sports Medicine Guidelines (ACSM) for Exercise Testing and Prescription; 3) AR 215-1, Moral, Welfare and Recreation Activities; 4) AR 600-63, Army Health Promotion. The provision at FAR 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition. Offeror shall include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications-Commercial Items with its offer. The clause 52.212-4, Contract Terms and Conditions-Commercial Items applies to this acquisition. The following additional FAR clauses are attached to this provision: 52.217-8, Option to Extend Services; 52.217-9, Option to Extend the Term of Contract. The clause 52.212-5, Contract Terms and Conditions Required to Implement Statues or Executive Orders?Commercial Items, applies to this acquisition. The additional FAR clauses cited in this clause are applicable: 52.222-21, Prohibition of Segregated Facilities; 52.222-26, Equal Opportunity; 52.222-35, Affirmative Action for Veterans of the Vietnam Era; 52.222-36, Affirmative Action for Workers with Disabilities; 52.222-37, Employment Reports on Special Disabled Veterans, Veterans of the Vietnam Era and Other Eligible Veterans; 52.222-19, 52.232-33, Payment by Electronic Funds Transfer-Central Contractor Registration. The Department of Defense requires all contractors to be registered in the Central Contractor Registration (CCR) to receive solicitations, contract awards or payments.
