- NA This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. The solicitation number is DAKF06-02-T-0040 and is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Feder al Acquisition Circular 01-09. This will be a 100% small business set aside type (NAICS Code 511210/size standard 18M). RFQs shall be quoted in accordance with the following: CLIN 0001. Base Year Lease ? Pharmacy Prescription (Rx) Call-In System. Quan tity: 1 System. Location: Pharmacy Service, Evans Army Community Hospital, Bldg 7500, Fort Carson, CO. System must support between ten to fifteen telephone lines. System must at a minimum: (1) allow patients to access medication and educational info rmation to include proper use of medication, indications, and problems associated with medication usage; (2) be 100% compatible with the Composite Health Care System (CHCS); (3) be able to continue pharmacy processing and store refill information while CHC S is unavailable, with the capability to automatically download the refill information when CHCS is available; (4) be able to notify patients in the event of medication recall; system must be able to initiate an emergency notification system, whereby staff members will be informed within a one-hour period of medication alerts; (5) be able to refill prescriptions over the Internet; (6) be able to notify the patient of prescription pickup (7) provide maintenance service; (8) provide operational training. C LIN 0002: Installation. Period of Performance: Award effective date through 30 September 2003. CLIN 0003: Second Option Period Lease ? Pharmacy RX Call-In System. Quantity: 1 System. Location: Pharmacy Service, Evans Army Community Hospital, B ldg 7500, Fort Carson, CO. System must at a minimum: (1) allow patient to access medication and educational information to include proper use of medication, indications, and problems associated with medication usage; (2) be 100% compatible with the Comp osite Health Care System (CHCS); (3) be able to store refill information while CHCS is unavailable, with the capability to automatically download the refill information when CHCS is available; (4) be able to notify patients in the event of medication recal l; system must be able to initiate an emergency notification system, whereby staff members will be informed within a one-hour period of medication alerts; (5) be able to refill prescriptions over the Internet; (6) be able to notify the patient of prescript ion pickup; (7) provide maintenance service; (8) provide operational training; (9) provide license renewal. Period of Performance: 1 October 2003 through 30 September 2004. Option to purchase does not apply. Payments shall be made on a monthly basis, u sing Government purchase card. FOB Destination delivery will be to Pharmacy Service, Evans Army Community Hospital, 7500 Cochrane Circle, Bldg 7500, Fort Carson, CO. The provision at FAR 52.212-1 Instructions to Offerors ? Commercial Items and FAR 52.21 2-4 Contract Terms and Conditions ? Commercial Items are applicable to this solicitation. Offerors shall include with their quotations a completed copy of the provision at FAR 52.212-3 ? Offeror Representations and Certifications ? Commercial Items. The provision at FAR 52.212-2 Evaluation ? Commercial Items is applicable to this solicitation. The following evaluation factors will be used to evaluate offeror?s quotation, and are listed in the relative descending order of importance: (1) technical capabi lity of the system offered to meet the Government requirement; (2) past performance and (3) price. Product literature shall be provided with the quotation and will be evaluated as part of the technical capabilities by the Government. The Government will evaluate the quality of the offeror?s past/present performance and the assessment of past/present performance will be used in two ways. The assessment of the offeror?s performance will be used as one means of evaluating the credibility of the offer?s quotation. A record of marginal or unacceptable past or present performance may be considered an indication that the representations made by the offeror are less than reliable. Such an indication may be reflected in the overall assessment of the offeror?s quotation. Second, the assessment of the offeror?s past or present performance will be used as one means of evaluating the relative capability of the offeror a nd the other competitors to meet the performance requirements of the proposed contract. Thus, an offeror with an exceptional record of past/present performance may receive a more favorable evaluation than another whose record is acceptable, even though bo th may have otherwise equally acceptable proposals. In investigating an offeror?s past/present performance, the Government will consider information from past and present customers, both inside and outside the Federal Government. Past/present performanc e data in the form of a questionnaire will be solicited from three sources selected by this office. Evaluation of past performance will be a subjective assessment based on a consideration of all relevant facts and circumstances. It will not be used on ab solute standards of acceptable performance. The Government is seeking to determine whether the offeror has consistently demonstrated a commitment to customer satisfaction and timely delivery of quality goods and services, of similar kind and complexity, a t fair and reasonable prices. This is a matter of judgment. Offerors may be given an opportunity to address especially unfavorable reports of past performance, and the offeror?s response or lack thereof will be taken into consideration. By past/present performance, the Government means the offeror?s record of conforming to Government specification requirements and to standards of good workmanship; the offeror?s reputation for reasonable and cooperative behavior and commitment to customer satisfaction; an d generally, the offeror?s business-like concern for the interests of the customer. Clause 52.212-5 (Dev) Contract Terms and Conditions Required to Implement Statutes or Executive Orders ? Commercial Items is hereby incorporated by reference. In addition , the following clauses are applicable and incorporated by reference: 52.222-21 Prohibition of Segregated Facilities, 52.222-26 Equal Opportunity, 52.222-35 Affirmative Action for Disabled Veterans of the Vietnam Era, 52.222-36 Affirmative Action for Work ers with Disabilities, 52.222-37 Employment Reports on Disabled Veterans and Veterans of the Vietnam Era, and 52.232-36 Payment by Third Party.
