Description

General Notice: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and written solicitation will not be issued. This solicitation F73HOS21890300 is issued as a request for quotation (RFQ). This solicitation document, incorporates provisions and clauses in effect through Federal Acquisition Circular (FAC) 2001-09. This acquisition is unrestricted. Quotes are to be received by this office NLT 1400 CST on 20 Sep 02. Quotes can be faxed, or e-mailed to the points of contacts listed on this notice. Contract Line Items (CLINs): 0001 Preventative maintenance for MVP RAD/FLUORO X-RAY EXPOSURE ROOM, SYSTEM # 501988RF10. 0002 MVP RAD/FLUORO X-RAY EXPOSURE ROOM SYSTEM # 501988RF12. Contract Parameters for the GE MVP-100 Rad/Fluoro X-Ray System are as follows: (A) Service Monday through Friday, 0800 to 1500 hours. (B) 1 Preventative maintenance and calibration visit open year. (C) 2 hour telephone response and 24 hour in-house response time. (D) Unlimited telephone assistance. (E) Contract to include all parts, labor, and travel expenses (not to include Image Intensifier and X-Ray Tube Head). (F) All service technicians must be manufacturer certified. Equipment Schedule is as follows: MVP-100 Rad/Fluoro X-Ray Exposure Room, Manufactured by General Electric, Serial numbers 11630E51 and 14735ESE, Model number MVP-100.Special Requirements: (1) Service personnel shall be fully qualified to repair and maintain the MVP-100. General Electric service school as required. (2) The contractor is required to maintain the below listed materials within reasonable proximity to the job-site, to travel with service personnel or to arrive on the jobsite no later than 24 hours after a specific requirement has been identified. (a) All manuals, schematics, handbooks, technical data and factory service updates to include all diagnostic and operating software required to troubleshoot repair and maintain the GE MVP-100 X-Ray Exposure Room as listed in the equipment schedule. (b) All exposure test equipment necessary to calibrate align, troubleshoot and repair the GE MVP-100 to include extender cards, ribbons, and other specialized tools as required. (c) All parts necessary to maintain the GE MVP-100 in a fully operational condition, including but not limited to electronic circuit cards, discrete components, and major assemblages. Required Service Dates: 01 Oct 02-30 Sep 03 FOB: Little Rock AFB AR 72099 BASIS of Award: Award will be made to the lowest price Offeror best demonstrating the ability to satisfy the technical capabilities of the services specified.
