Description
This is a combined synopsis/solicitation for commerical services prepared in accordance with format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation is issued as a Request for Quotation (RFQ), solicitation number F05611-02-Q-0701, in accordance with FAR Parts 12 & 13. The North American Industrial Classification System (NAICS) number is 813110 and the business size is 500 employees. The proposed acquisition is reserved for small business. LINE ITEM 0001: Conduct 42 weekend Masses. LINE ITEM 0002: Conduct 14 Rite of Reconciliations, LINE ITEM 0003: Conduct 50 Daily Masses. LINE ITEM 0004: Option year one. LINE ITEM 0005: Option year two. LINE ITEM 0006: Option year three. LINE ITEM 0007: Option year four. The FOB for this RFQ is Destination. Quotations receiced with other than FOB Destination pricing will not be considered. The Statement of Work is as follows: STATEMENT OF WORK AUXILIARY CATHOLIC CHAPLAIN QUALIFICATIONS: 1. Written endorsement reflecting good standing in a Roman Catholic Diocese or religious community that is acknowledged by the Archdiocese for the Military Services USA. Must furnish a resume showing education, experience, and personal and professional references. References must include a name and phone number. The resume, or attached document to the resume, must provide sufficient information to determine whether requirements identified in the Statement of Work. 2. Contractor agrees to conduct himself with due regard to public conventions and morals, and further agrees not to do or commit any act or thing that will reasonably tend to degrade him or to bring him or the Air Force Chaplaincy into public hatred, contempt, scorn, or ridicule, or that will reasonably tend to shock, insult or offend the community or offend public morals or decency, or to prejudice the Air Force Chaplaincy. Contractor shall not be arrested or violate any applicable Air Force Instruction. RESPONSIBILITIES: 1. MASSES: Provide daily, weekend and holy day Masses as requested by the active duty Catholic chaplain. 2. HOSPITAL PATIENT VISITATION: The Contractor will provide visitation to Catholic parishioners who are inpatients at local hospitals only in the absence of the active duty Catholic chaplain and request of the Wing Chaplain. 3. RITE OF RECONCILIATION: The Contractor will provide for the celebration of the Rite of Reconciliation prior to the Saturday evening service only on the weekends the contractor is presiding over Mass. 4. WEDDINGS, BAPTISMS AND FUNERALS: Weddings, Baptisms, and Funerals will be performed by the contractor only with the prior coordination, assignment, and approval of the active duty Catholic chaplain. The active duty Catholic chaplain is not obligated to approve or assign any of these ceremonies or rehearsals. 5. EXTENDED PRIEST ABSENCE COVERAGE: The contractor could be requested to provide Masses, Rite of Reconciliation, Religious Education classes, Weddings, Funerals and Baptisms in the event that the active duty Catholic chaplain is absent or deployed for an extended time period. Scheduling of these services will be coordinated and approved by the Wing Chaplain. However, the Wing Chaplain is not obligated to approve or assign these services to the contractor for this period. 6. PAYMENT FOR SERVICES: Payment shall be made monthly, in arrears, upon submission of a proper invoice. Invoices will be submitted to the Catholic Chaplain for certification prior to submission to the NCOIC of Financial Management. Offerors must comply with all instructions contained in FAR 52.212-1, Instructions to Offerors-Commercial Items, applies to this acquisition and not addenda applies. The provisions at FAR 52.212-2, Evaluation of Commerical Items applies to this acquisition. The Government will award a contract resulting from this solicitation to the responsive, responsible offeror whose technically acceptable offer conforming to the solicitation will be most advantageous to the government, price and other factors considered. The Clause 52.213-3,Contract Terms and Conditions-Commerical Items, applies to this acquisition. The Clause 52.212-5,Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commerical Items applies to this acquisition, no addenda applies. Additional FAR clauses cited in the Clause appicable to this acquisition are: (b) 12,13,14,15,24. The Clauses 52.217-8, Option to Extend Services applies to this acquisition. The Clause 52.217-9, Option to Extend the Terms of the Contract applies to this acquisition. DFARS 252.212-7001, Contract Terms and Conditions Required to Implement Status of Executive Orders applicable to Defense Acquisition of Commerical Items Applies to this acquisition with the inclusion of the follwoing DFARS: 252.204-7004, Required Central Contractor Registration, 252.225-7001, Buy American Act and Balance of Payments Program.
