Description
This is a combined synopsis/solicitation for commercial items, prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is NHLBI-PS-2002-144, and is issued as a request for proposal (RFP). The solicitation/contract will include all applicable provisions and clauses in effect through the Federal Acquisition Circular 2001-08. There are no set aside restrictions for this acquisition. The North American Industry Classification System (NAICS) Code is 339112, size standard is 500 employee. It is the intent of the National Heart, Lung, and Blood Institute to purchase a Echocardiography System Model Vivid 7, GE Medical Systems brand or equivalent, manufactured by GE Medical Systems, Waukesah, Wisconsin. The purchase includes: (1) High Performance All Digital Ultrasound Base System, Model Vivid 7 BT02 110VAC, Catalog Number H4800SN, quantity 1 each. (2) Transducer, Wide Band, Multi-Frequency Active Matrix Phased Array Probe, Model Matrix Array 3S Probe, Catalog Number H45011SZ, quantity 1 each. (3) Transducer, Wide Band, Multi-Frequency Active Matrix Phased Array Probe, Model 10S 4-11 MhZ Probe, Catalog Number H4901PC, quantity 1 each. (4) Vivid & Echo DICOM, Catalog Number H45001YW, quantity 1 each. (5) EPPC Strain Imaging with Q-Analysis, Catalog Number H45521FJ, quantity 2 each. (6) V7 Stress Echo Package with Q-Analysis , Catalog Number H4800LZ, quantity 1 each. (7) ECHOPAC Stress With Q-Analysis, Model H4800SD, quantity 2 each. (8) Strain Imaging with Q-analysis, Catalog Number H4800SE, quantity 1 each. (9) EchoPAC P Workstation, Model EPPC BT02 Workstation 110 VAC, Catalog Number H4800WS, quantity 2 each. (10) One year standard service warranty agreement. The place of delivery and acceptance is the National Institutes of Health, National Heart, Lung, and Blood Institute, Bethesda, Maryland 20892, the desired F.O.B. Point is Origin. The Government's desired delivery schedule is ninety (90) days after contract award. The provisions of FAR Clause 52.212-1, Instructions to Offerors-Commercial Items, applies to this acquisition. The provisions of FAR Clause 52.212-2, Evaluation - Commercial Items is applicable to this acquisition. Award will be made to the lowest priced, technically acceptable offeror meeting the Government's specifications. The offeror must include a completed copy of the provision of FAR Clause 52.212-3, Offeror Representations and Certifications - Commercial Items with its offer. The provisions of FAR Clause 52.212-4, Contract Terms and Conditions - Commercial Items, applies to this acquisition. The Offeror must provide descriptive literature inclusive of specifications as part of the proposal. The FAR Clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items, applies to this acquisition and the FAR clauses cited are applicable to this acquisition. The purchase includes the trade-in of a Philips Echo Machine, Model ATL HDI 5000, serial number 010989. The trade-in unit value is $33,000. There are no Commerce Business Daily numbered notes applicable to this requirement.
