- This is a combined synopsis/solicitation for commercial service prepared in accordance with the format in FAR Subpart 12.6 and 13.5 as supplements, with the additional information included in this notice.=20 This announcement constitutes the only solicitation. This is a Request For Quotation and a written solicitation will not be issued.=20 Solicitation number RFQ 667-22-03 will be awarded using the simplified acquisition procedures. This solicitation document and incorporated clauses and provisions are those in effect through Federal Acquisition Circular 2001-09. The North American Industry Classification System (NAICS) number is 561320, annual gross receipts $11.5 million. 1.=20 Furnish 1 full-time equivalent (FTE) ARRT Radiation Oncology Technologist to perform the requirements of this contract approximately 2080 hours. An FTE is defined as a contractor=92s employee providing service 8 hours per day, 40 hours per week, 52 weeks per year. The price shall be all inclusive to include travel, lodging, car rental, malpractice, security check and all related expenses with providing.=20 REQUIREMENT: The ARRT Services: 1. The contract resulting from this solicitation shall be to provide 1 ARRT with experience in Radiation Oncology Services at the Overton Brooks VAMC, herein called VAMC, 510 East Stoner Avenue, Shreveport, Louisiana 71101. The contractor agrees, in accordance with the terms and conditions stated herein, to furnish the services specified and at the prices specified in this contract.=20 The initial listings of services to be furnished shall be identified in the schedule. Services shall include, but are not limited to the following: Performs quality control and quality assurance procedures on linear accelerator and simulator. Performs simulations and mark the patient=92s treatment field. Performs radiation therapy treatments according to the patient=92s prescription. Documents daily treatments.=20 Assists in treatment planning CT scan. Makes blocks, if required in treatment. Takes iView portal films, as required. Enters treatment parameters into Radiation Therapy Desk Top Computer. CREDENTIALS:=20 Registered in Radiation Therapy by The American Registry of Radiologic Technologists (ARRT). Licensed in the State of Louisiana or Territory of the US as a Radiation Therapy Technologist. Minimum of one year experience. LOCATION OF WORK AND FACILITIES: Radiation Oncology Service (BE38), Overton Brooks VA Medical Center, Shreveport, Louisiana 71101. The Service has the following equipment: Elekta Linear Accelerator with 6 and 15 MeV photons and 6, 8, 10, 12, and 15 MeV electrons; Multileaf Collimator (MLC); and iView Electronic Portal Imaging. Varian Ximatron Simulator. Elekta Precise Treatment Planning System. WORK SETTING: Technical and clinical staff includes one full time radiation therapist and one part time therapist, one physicist, one radiation oncologist, and one nurse. Average number of treatments per day is 28-32. Average number of simulations per week is six. 2.=20 The payments for any leave, including sick leave, holiday pay or vacation time, are the responsibility of the contractor. Payment for security check is the responsibility of the contractor. The contractor shall reimburse the VA for the security check of each temporary employee sent to perform under this contract. This fee is $200 per individual. 3. Provide technical and professional direction of post-operative care of patients in Post Anesthesia Care Unit, on Surgical Wards and the Surgical Intensive Care Unit, in conjunction with the Surgical Staff. 4. Authority: This procurement is executed under Enhanced Sharing Authority, Public Law 104-262, Title 38 U.S.C. 8153. 5. TERM OF CONTRACT: a. This contract is effective for the period 10-7-02 through 9-30-03 for one FTE. The contract is subject to the availability of VA funds. No service shall be performed by the contractor after September 30, 2003. 6. WORK HOURS: a. Regular tour of duty, Monday through Friday, 8:00 am to 4:30 pm. No call and no weekend work. A 30 minute lunch break is allowed. b. National Holidays: The ten (10) holidays observed by the Federal Government, i.e., New Years Day, President's Day, Martin Luther King Jr.'s Birthday, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, and Christmas Day. 7. PAYMENTS: Sums due for actual work done, the Contractor will be paid monthly, in arrears, upon receipt of a properly prepared invoice FAR 52.232-25.=20 The Contractor shall submit all invoices to the following address: Chief Fiscal Service (04), Overton Brooks VA Medical Center, 510 East Stoner Ave., Shreveport, LA 71101. 8. PERSONNEL POLICY: The contractor shall be responsible for providing: - Health examinations. - Worker's compensation. - Professional liability insurance. -=20 Income tax withholdings. - Social security payments. The parties agree that the contractor shall not be considered a VAMC employee for any purpose. 9. CONTRACT PERFORMANCE: a. The contractor will be monitored through a variety of mechanisms, including but not limited to: - Clinic appointment lists showing patients seen. - sign-in/out log. - Chart documentation of patient diagnosis, treatment and or follow-up procedure. - QA monitors designed to assess the quality of care provided in the clinic. b. The Chief of Radiation Oncology or his designee will be responsible for monitoring the quality of services provided under this contract. 10.
