NA This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations a re being requested and a written solicitation will not be issued. This solicitation DADA13-02-T-0096 is being issued as a Request for Quotation. This solicitation document and incorporated clauses/provisions are those in effect through Federal Acquisition Circular 2001-09. NAICS Code is 325413 and the size standard is 500 employees. This solicitation is issued as full and open competition. The Western Regional Contracting Office intends to award a firm fixed price contract for the following commercial item( s) manufactured by International Remote Imaging Systems. CLIN 0001?540 each Catalog No: 800-0057 Chemstrip?Delivery Schedule 45 each month; CLIN 0002? 2 each Catalog No: 475-0001 Iris Spec 1 Calibrator/Control?Delivery Schedule 6 each month; CLIN 0003?12 e ach Catalog No: 475-0024 Iris Stain Composition-Delivery Schedule 1 each month; CLIN 0004?8 each Catalog No: 799-0007 Preventive Maintenance Kit-Delivery Schedule 2 each every 3 months;CLIN 0005?48 each Catalog No: 475-0020 Iris Spec Gravity Control 1.025- Delivery Schedule 4 each month; CLIN 0006?48 each Catalog No: 475-0010 Iris Spec Gravity Control 1.060-Delivery Schedule 4 each month; CLIN 0007?72 each Catalog No: 475-0003 Irisovel Cleaner-Delivery Schedule 6 each month; CLIN 0008?24 each Catalog No: 800-0058 Chemstrips/Calibration Strips-Delivery Schedule 2 each month; CLIN 0009?8 each Catalog No: 800-0039 Printer Ribbon-Delivery Schedule 2 each every 3 months Delivery schedule: Delivery of scheduled items shall occur the first week of each month after date of award. Period of performance: 01 October 2002 through 30 September 2003. All products shall have a 12 month shelf life upon receipt by the Department of P athology, if a product is received with a shelf life less than 12 months the product will be rejected and returned for credit. Quantities shown are based on an estimated anticipated usage. Ship to address is Madigan Army Medical Center, Receiving Section, Bldg 9040A, Tacoma, WA 98431. Request FOB Destination pricing. Method of payment will be the Government Purchase Card (VISA). BASIS OF SELECTION ? BRAND NAME. The manufacturer and product(s) configuration specified are acceptable for this procurement. Sele ction will be made based on price, technical capability, and past performance. Quoters will submit along with their pricing the following information for evaluation of quotes. A discussion of the company?s ability to obtain the product required under this request for quotation, and if the quoter is not an authorized distributor of the product describe, explain in detail where and how the company intends to obtain the product to meet the delivery schedule in regards to technical capability. Quoters shall als o provide a maximum of two references with whom recent past performance can be verified. Provide the scope of work, the contract number, dollar value, name, title, and a telephone number of the person with whom to speak regarding your performance related t o conformance with contract requirements; standards of good workmanship; adherence to contract schedule; reasonable and cooperative behavior and; commitment to customer satisfaction. The Government as part of the evaluation process may consider information that is obtained from other sources. NOTE: Quotes submitted which do not include a written technical representation that addresses the evaluation factors may not be considered for selection. Although price is considered significantly more important when c ompared to technical capability and past performance, the Government reserves the right to select a quote other than the lowest price. Offers that fail to furnish required representations or information, or reject the terms and conditions of this solicitation may be excluded from consideration. Quoters are responsible for accurate addresses and t elephone numbers for submitted past performance information. Product not clearly described will not be considered. Product otherwise equal may not satisfy the requirement due to differences in environmental application, etc.
