- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulations document (FAR) subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Proposals are being requested and a written solicitation will not be issued. Solicitation number 9TER-02-FDH-0060 is issued as a Request for Proposal (RFP). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC)02-09. Supplements can be accessed at www.eps.gov. The simplified acquisition procedures of FAR Part 13 are applicable to this procurement. The objective of this contract will be the provision of the following: Surgical Scheduling System with patient Charting, Scheduling, Historical time averaging, Case logging, Case costing, Inventory management, Surgeon preference cards, Statistical Reporting, Installation and Training at Luke Airforce Base, 10 licenses, 1 year software support contract, and onsite installation and training. See the attached statement of work and cover letter for specifics regarding the requirement and the method of award evaluation criteria. Type of contract: A firm fixed price definite quantity, definite delivery contract shall be awarded. All prices are to be quoted ?FOB Destination.? Please acknowledge all requirements. The provision at FAR 52.212-1, Instructions to Offerors, applies to this acquisition. Offerors shall include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications Commercial Items, with its offer. The clause at FAR 52.212-4, Contract terms and Conditions Commercial Items, applies to this acquisition. The clause at FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Commercial Items, applies to this acquisition. The prospective awardee is required to register in IT Solutions Shop at it-solutions.gsa.gov. for post-award processing. Proposals received by 4:00 p.m., PST, 9/26/02 will be considered by the government. All pre-proposal questions must be received via email by Noon (12pm) on Tuesday 9/24/02 to allow time for distribution of answers prior to the proposal due date.
