- Description
- (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) This purchase request (PR) will be issued as a request for quotation (RFQ) under the following PR/solicitation number: F5SGSL22040200. (iii) This PR and the provisions and clauses incorporated are those in effect through Federal Acquisition Circular (FAC) 2001-09. (iv) This acquisition is set-aside 100% for small businesses in regards to size standards. The applicable NAICS is 622110, and the small business size standard is $5,000,000. (v) This RFQ contains five (5) line item(s): 0001, Certified Medical Coding Services (Basic Year, 1 Oct 2002 - 30 Sep 2003) ; 1001, Certified Medical Coding Services (Option Year One, 1 Oct 2003 - 30 Sep 2004; 2001, Certified Medical Coding Services (Option Year Two, 1 Oct 2004 - 30 Sep 2005; 3001, Certified Medical Coding Services (Option Year Three, 1 Oct 2005 - 30 Sep 2006; 4001, Certified Medical Coding Services (Option Year Four, 1 Oct 2006 - 30 Sep 2007. Total amount awarded will be split up into 12 monthly payments. (vi) Certified Medical Coding Services: See attached Statement of Work (SOW) for a complete description of the requirement or contact Darlyne Pangelinan (see paragraph xvii). (vii) The performance period will be 1 Oct 2002 - 30 Sep 2003 with 4 option years. Place of performance is 15 MDG, 755 Scott Circle, Hickam AFB, HI 96853. Acceptance of services will be performed by the Government. (viii) The provision at 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition. (ix) Offers will be evaluated using the following evaluation factors: The Contracting Officer may evaluate quotations or offers based on price alone or price and other factors. Evaluation of other factors does not require the creation or existence of a formal database, but may be based on such information as the Contracting Officer?s knowledge, previous work experience (past performance), customer surveys, etc. The contract will be awarded to the offeror that provides the best value to the government. (x) All offers must include a completed copy of the provision at 52.212-3 , Offeror Representations and Certifications -- Commercial Items. A copy of the provision may be downloaded from the web-site stated in paragraph (xiii).
