Description

NA This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals a re being requested and a written solicitation will not be issued. DAJA22-02-T-0494 is a request for quotation (RFQ); its document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 2001-09. For procurement are ITEM 0001: Tumbling MAT, as specified below ? 198 EA, priced at F.O.B. destination to containerization point, or FOB Origin. Specify and provide address, point of contact, telephone, fax numbers and email address. PolyEthylene-Foam, high impact, highly shock absorbent Folding Mat Velcro on all Four (4) Sides Color: BLUE DIMENSIONS: 1 M x 2 M x 4 CM (3'3inches x 6'6inches x 1.5inches) to 1.524 M x 3.048 M x 5.08 CM (5' x 10-' x 2inches) Thickness may vary from 1-3/8 inches to 2 inches ATTENTION: OFFERORS ARE REQUIRED TO IDENTIFY OFFERED PRODUCT (MAKE AND MODEL) AND SUBMIT SUFFICIENT TECHNICAL LITERATURE WITH OFFER TO ENABLE A TECHNICAL EVALUATION OF MAT SPECIFICATIONS. DELIVERY SCHEDULE. Provide expected delivery time either to FOB Destination (container point may be New Cumberland) or FOB Origin. Point of Contact for Delivery and further instructions will be provided upon award. The following provisions in their latest editions apply to this solicitation: (1) FAR 52.212-1, Instructions to Offerors with the addendum that: ?The offer submitted shall include: (1) Proposed prices; (2) Literature or brochures to show that the specifications are met; (3) completed copy of FAR 52.212-3,Offeror Representations and Certifications-Commercial Items; and (4) Cage Code, DUNS and TIN; (2) FAR 52.212-2, Evaluation , with the following addendum for the basis of award: Award will be made to the responsive low offeror with technically acceptable offer. Offeror without technical literature or with insufficient technical literature to show that offered product meets the specification shall not be evaluated for award. (3) FAR 52.212-3, Offerors Representations and Certifications; a completed copy of which shall be submitted with the offeror (4) FAR 52.212-4, Contract Terms and Conditions, with addendum for site specific payment procedures and tax relief clause; and (5) FAR 52.212-5 (Deviation) , Contract Terms and Conditions Required to Implement Statutes or Executive Orders .
