Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation is issued as Request for Quotation (RFQ) No. F49642-02-T-0305. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2001-09. This is a small business set-aside. The North American Industry Classification System code is 235110 (formerly SIC code 1711) at $12.0 million size standard. A complete price breakdown is required as part of your offer for each line item listed above. The following is an example: number of hours, hourly rate, materials, completion time for each building, etc. ?Notice to Offerors: Funds are not presently available for this project. No award will be made under this solicitation until funds are available. The Government reserves the right to cancel this solicitation, either before or after the closing date. If requirement is partially funded, it will be ?partially? awarded.? INSPECTON AND ACCEPTANCE TERMS: Supplies/services will be inspected by Quality Assurance personnel and accepted at job site. DESCRIPTION/SOW: PART I STATEMENT OF WORK SWI-01 Contractor to furnish all plant, labor, equipment, materials and appliances to perform all operations necessary to thoroughly clean Kitchen Exhaust Hood Systems and Vent Duct Systems; at various Bldgs on Bolling AFB. SWI-02 Contractors shall label all hoods filters and ducts. Labels shall show date, contractor?s name and the time of cleaning, in accordance with the National Fire Protection Agency ? National Fire Code Regulations. PART II SPECIAL CONDITIONS SC2-01 Project Manager: The contractor shall provide a project manager or person who shall be responsible for the competent performance of the work, and shall have full authority to act for the contractor. The project manager shall also be able to read, write, speak and understand English. SC2-02 Employee: Contact personnel shall present a neat appearance and be easily recognized while on base. This may be accomplished by wearing distinctive clothing bearing the name of the company, or by wearing appropriate badges which contain the company name and employee name. The contractor shall be responsible for acquiring an appropriate number of badges to meet his/her needs at the contractor?s expense. The contractor shall also ensure that employees have valid/current driver?s license as applicable to operate equipment in accordance with state, county and local laws and regulations. The abrupt absence of contractor at any time shall not constitute an excuse for nonperformance under this contract. SC2-03 Quality Assurance: The government will monitor the contract?s performance under this contract using the methods of surveillance as specified. a. Monitoring will be performed by a government Quality Assurance. b. The contractor shall coordinate date and time to arrive and perform required tasks. c. The contractor shall respond within 4 hours for re-cleaning after notification by the QA?(s) of unsatisfactory work. SC2-04 Performance Evaluation Meetings: The project manager shall meet with the QA and the contracting officer during the first month of the contract. Meetings will be as often as necessary thereafter, as determined by the contracting officer. However, a meeting will be held whenever a Discrepancy Report is issued. A mutual effort will be made to resolve all problems identified. The contractor?s manager, contracting officer, and QA will sign the written minutes of these meetings. Should the contractor not concur with the minutes, his company representative will state in writing to the contracting officer any areas of non-concurrence. SC2-05 Hours of Operation: a. Normal Hours: The contractor shall maintain the following customer service hours: 0430 hrs ? 2100 hrs. b. Recognized Holidays: The contractor is not required to provide normal service on the following days: New Years Day President?s Day Martin Luther King Jr.?s Birthday Memorial Day Independence Day Labor Day Columbus Day Veteran?s Day Thanksgiving Day Christmas Day SC2-06 Contractor?s Equipment: Contractor?s furnished equipment shall conform to all state, county and local laws; and comply with all OSHA/Safety rules and Base Regulations. SC2-07 Contractor Furnished Items and Services: Except for those items or services specifically stated to be government furnished in Section SC2-08, the contractor shall furnish everything required to perform this Statement of Work. SC2-08 Government Owned Equipment/Property Cleaning: All exhaust hoods and ducts listed in TP3-06. TECHNICAL PROVISIONS TP3-01 Contractor shall remove all grease accumulation from kitchen exhaust hoods systems. TP3-02 Contractor shall clean all dryer vent ducts from dryer connection point to exterior of the building. TP3-03 Cleaning: All the surfaces (exhaust hoods, fans, exterior walls, cupolas, roofs, louvers, etc.) shall be cleaned by the following typical methods: hand scraping, scrubbing, brushing, air agitation, vacuum cleaning, etc. Fans may be steamed. Degreasers may be used or any other detergents. TP3-04 Contracting shall use drop cloths where necessary or at the direction of the Contracting Officer or QA(s). TP3-05 Contractor shall label all hood filters and ducts to read date completed and contractor?s name in accordance with the National Fire Protection Agency National Fire Code Regulations. TP3-06 Cleaning of hoods and duct systems to be accomplished as indicated for each building. NOTE: Building 50, 2482, 2565, 4514, 4456 and 1310 shall be serviced six times a year during the months of October, December, February, April, June, and August. TP3-07 Cleaning of hoods and duct systems to be accomplished as indicated for each building. NOTE: Buildings 3618, 3619, 3620 and 523 shall be serviced six times a year during the months of November, January, March, May, July and September TP3-08 Cleaning of hoods and duct systems to be accomplished at Buildings 1301 and 4500, twelve times a year during the months of October, November, December, January, February, March, April, May, June, July, August, and September. TP3-09 Buildings to be serviced: P-50: Officer?s Club ? Four hoods and ducts over ranges, broilers and grills ? 6 times a year. Building 523: Base Laundromat ? dryer vents behind east wall ? 6 times a year. TP3-010 Clean hoods and ducts over ranges, broilers and grills in Building 4456, Child Care Center - 6 times per year, October, December, February, April, June, and August. TP3 ? 011 Clean dust collectors in Building 4472, Arts & Crafts - 2 times per year, November and September. TP3 ? 012 Clean HVAC systems in Building 370, Civil Engineering Squadron, once a year, in the month of November. TP3 ? 013 Clean HVAC systems in Building 371, Civil Engineering Squadron, once a year, in the month of November. 1302: Blanchard Barracks ? Laundry rooms first through ninth floors, north and south wings, dryer vent system ? 3 times per year December, April and August. 1310: Base Bowling Lanes ? One hood and duct over grill, deep fryer ? 4 times per year. November, January, March and May. 2565: NCO Club ? Two hoods and ducts over ranges, boiler and grills ? 6 times per year. 2482: NCO Club Slip Inn ? Two hoods and ducts over ranges, and grills 4 times per year. 3621: Mathis Manor ? Laundry rooms, ground floor, dryer vent systems ? 2 times per year February and August. 3623: Cheshire Dorm ? Two hoods and duct over grill ? 3 times a year December, March, and July. 4514: Base BX Snack Bar ? Two Hoods and duct over range and grill ? 4 times per year. 3618: Temporary Lodging Facility ? First and second floor dryer vent systems ? 6 times per year, October, December, February, April, June and August. 3619: Temporary Lodging Facility ? First and second floor dryer vent systems ? 6 times per year, October, December, February, April, June and August. 3620: Temporary Lodging Facility ? First through third floors dryer vent systems ? 6 times per year, October, December, February, April, June and August 4500: Burger King - Two hoods and ducts over range grill and fryer ? 12 times per year October, November, December, January, February, March, April, May, June, July, August and September 1301: Base Dining Hall ? Ten hoods and ducts over ranges, boilers, steam tables and grills, twelve times per year. COORDINATION: Prior to start of work the contractor shall contact Ms. Helen Walker at (202) 767-1259, building 370. FAR PROVISIONS: The provisions at 52.212-1, Instructions to Offerors-Commercial Items and 52.212-3, Offerors Representation and Certifications-Commercial Items are hereby incorporated. Addendum to FAR Clause 52.212-1 applies to this acquisition is as follow: 52.237-1, 52.252-1, 52.252-5: (b) DFARS (Chapter 2), 252.212-7000 and 252.225-7000. Add paragraph (l), ?Offeror must include, with its offer, a completed copy of provision FAR 52.212-3 in full text.? Clause 52.212-2, Evaluation?Commercial Items applies to this solicitation, specifically the following criteria: The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforms to the solicitation and price is determined most advantageous to the Government.
