Description
This is a combined synopsis / solicitation for commercial items prepared in accordance with FAR 13 and the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the ONLY solicitation; quotes are being requested and A SEPARATE WRITTEN SOLICITATION WILL NOT BE ISSUED. Solicitation number / RFQ N00244-02-T-1360 applies; an electronic copy of the RFQ can be accessed at http://www.neco.navy.mil. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-09 and DFAR Change Notices effective through 07/30/02. The standard industrial code is 8322, NAICs #624229 and the business size standard of $6.0 million is applicable. This requirement is for a firm fixed priced contract. IMPORTANT NOTICE: DFARS 252.204-7004 ?Required Central Contractor Registration? (CCR) is required and applies to all solicitations issued on / or after 06-01-98. Lack of registration in the CCR database will make an offeror/quoter INELIGIBLE FOR AWARD of a contract/purchase order. Please ensure compliance with this regulation when submitting your quote. This announcement is published as a Total Small Business Set-Aside action per FAR 52.219-1 and 52.219-6 (this means you must be a small business, quoting the product of a small-business manufacturer); all eligible, responsible sources may submit an offer. The Fleet and Industrial Supply Center, San Diego (FISC-SD) is announcing a requirement for the following: Personal Growth Retreats (72 hours). Retreat facilities are required within the immediate San Diego, CA locale. The following dates are required: (This schedule is not negotiable) 14-17 November 2002, 5-8 December 2002, 9-12 January 2003, 6-9 February 2003, 6-9 March 2003, 3-6 April 2003, 1-4 May 2003, 5-8 June 2003, 7-10 August 2003, 4-7 September 2003. All services will be inspected and accepted at Origin (Contractor Location). The following Statement of Work (SOW) is hereby applicable and sets forth the minimum requirements that the US Government will accept for the required services. REQUIREMENTS LIST: Contractor is to provide lodging, meals and use of facilities exclusively for CREDO San Diego for ten (10) Personal Growth Retreats (PGR); in the San Diego, CA area. SPECIFICATIONS: The retreat area must be accessible to a forty-four, passenger bus. Medical emergencies will be handled by the CREDO (NAVY Staff), in coordination with the retreat site personnel to determine the proper cour se of action. When possible military personnel and their family members will be treated by ?On Duty? medical personnel. The retreat site must have staff personnel available throughout the retreat period. The Director of CREDO San Diego, must be notified two (2) weeks in advance of any disruption of retreat facilities that may affect the ability of the retreat site to properly fulfill its obligations to this contract. Telephone services must be available for CREDO Staff use. Heating and ventilation must be kept at a comfortable level, as determined by the CREDO Staff at all times during retreats. There must be a fire pit available for use, including firewood. All facilities must be clean, neat and ready upon arrival. LODGING REQUIREMENTS: Shall be made available for participants. Not to exceed thirty-five. CREDO San Diego will provide a ?Not Less than? number of par ticipants no later than forty-eight hours prior to each retreat. A single site with exclusive use of berthing and meetings. Separate berthing areas for men and women; including bathing, showering and toilet facilities must be sanitary and in good working order. A separate berthing area is required for the chaplain and up to five team members. All beds shall be complete with linens, blankets and pillows. Each room must have adequate space for personal belongings. The retreat site must allow for personal solitude and time in a confidential atmosphere throughout the retreat. Facility shall include a room capable of seating thirty-five, with at least one electrical outlet and two (2) additional meeting areas (indoors) large enough to accommodate ten (10) people. If a fireplace is in any meeting room, firewood shall be included. Recorded music will be played throughout the day and evening from inside the common meeting room. FOOD REQUIREMENTS Provide a snack each evening beginning at approximately 7:15PM consisting of cake or cookies; an alternative for dieters, such as fruit and raw vegetables; coffee (regular and decafinated), fruit juices or punch, tea, hot chocolate, cold juices and water must be available at all times. The snack on the 3rd day of the PGR must be graham crackers, marshmallows and chocolate bars. MEAL SCHEDULE: PERSONAL GROWTH RETREAT 1ST Day 7:15PM Snack 2nd day 7:30 - 8:00AM Breakfast 12:00 - 12:30PM Lunch 6:00 - 6:30PM Dinner 7:15PM Snack 3rd day 7:30 - 8:00AM Breakfast 12:00 - 12:30PM Lunch 6:00 - 6:30PM Dinner 7:15PM Snack 4th Day 7:30 - 8:00AM Breakfast 12:00 - 12:30PM Lunch These times are approximate and should be f lexible within the hour at the discretion of the chaplain. On full session days three (3) balanced meals shall be provided. SAMPLE MENU: Snacks: Dessert and fresh fruit Breakfast: Fruit juice, milk (regular and low fat);cold cereal; toast; eggs; meat entr?e (bacon, ham or sausage);potatoes; fresh fruit; condiments for breads, butter, jam, jelly, or honey. Salt and pepper. Waffles or pancakes with syrup; fresh fruit. Lunch: Fruit juice, milk (regular and low fat); soup and salad with dressing choices; sandwiches with meat and cheese, lettuce, tomatoes and applicable condiments; french fries or potato chips; and dessert. Alternate Lunch Menu: Fruit juice, milk (regular and low fat); soup and salad of lettuce, peppers, celery, carrots, cheese, ham, eggs, radishes, broccoli, cauliflower, sprouts, tuna salad, croutons; fresh fruit; bread and butter; dessert. Dinner: Fruit juice, milk (regular and low fat); soup; salad with dressing choices; hot meal entr?e; potatoes; casserole; fresh vegetable, particularly green vegetables; fresh fruit, bread and butter. Evening Snack: Items that would normally be served as a dinner dessert. The following provisions and clauses are applicable and are hereby incorporated into the solicitation by reference and or by full text. Commercial Items-Alt I-Oct 1998 (reference) Note this provision is considered a ?fill-in.? All applicable fields must be completed. Submit this provision with your quote. Clause 52.212-4, Contract Terms and Conditions ? Commercial Items (reference) applies as well as the following addendum clauses: FAR 219-1, Small Business Program Representations; FAR 52.219-6, Notice of Total Small Business Set-Aside; FAR 52.211-5, Material Requirements. 52.232-18 Availability of Funds (full text) Funds are not presently available for this contract. The Government's obligation under this contract is contingent upon the availability of appropriated funds from which payment for contract purposes can be made. No legal liability on the part of the Government for any payment may arise un til funds are made available to the Contracting Officer for this contract and until the Contractor receives notice of such availability, to be confirmed in writing by the Contracting Officer. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders ? Commercial Items applies with the following applicable clauses for paragraph (b): FAR 52.219-14 Limitations on Subcontracting; FAR 52.222-26, Equal Opportunity; FAR 52.222-35, Affirmative Action for Disabled Veterans and Veterans of the Vietnam Era, FAR 52.222-36, Affirmative Action for Workers with Disabilities; FAR 52.222-37, Employment Reports on Disabled Veterans and Veterans of the Vietnam Era; 52.232-22 Payment by Electronic Funds Transfer-Central Contractor Registration. DFARS 252.212-7001, Contract Terms and Conditions Required to Implement Statutes or Executive Order s Applicable to Defense Acquisitions of Commercial Items, applies with the following clauses applicable for paragraph (b): 252.204-7004 Required Central Contractor Registration; DFARS 252.225-7007, Trade Agreements-Balance of Payment Program; and DFARS 252.225-7012, Preference for Certain Domestic Commodities. OFFERORS ARE REQUIRED TO COMPLETE AND INCLUDE A COPY OF THE FOLLOWING PROVISIONS WITH THEIR PROPOSALS: FAR 52.212-3, Offeror Representation and Certifications ? Commercial Items. The government intends to make a single award to the responsible offeror whose offer is the most advantageous to the government considering price and price-related factors. 52.212-2 EVALUATION OF COMMERCIAL ITEMS--OCT 1995 (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most adva ntageous to the Government, price and other factors considered. The following factors shall be used to evaluate the quote. (i) Capability of Contractor to meet and accomplish the requirement. To be determined acceptable, the contractor must furnish a current facility capability statement. This statement should list the type of housing available, food, and facilities as required by the SOW. (ii) Past Performance. Provide Past Performance data on your company?s two most recently completed Federal Government contracts (not to exceed three (3) years since completion) for like and similar services required by this solicitation. If you do not have Federal Government contracts, list state, local or commercial contracts. Provide contract number, date awarded, items/services provided, unit prices, identify the Government entity and/or commercial company, point of contac t/phone number/e-mail address. This performance data is to be used to evaluate your company?s past performance in meeting contract cost/price, technical and delivery objectives. (iii) Pricing. Contractors must provide current pricing for each CLIN specified. Provide any current commercially published price lists or catalog price listings for the services to be provided. Pricing will be evaluated against other quotes received. Only those quotes which have provided all required information for each criteria will be considered for award. Ability to meet the requirements set forth in this RFQ shall be demonstrated by the contractor?s submission of a signed quote which has provided all required information. Quotes which take exception to specified requirements and do not meet the Government?s requirement may be determined to be unacceptable and not considered for award. At the discretion of the Contracting Officer, the Government intends to evaluate quotes without discussions and award a Firm Fixed Price purchase order. Each initial offer should, therefore contain the Offeror?s best terms from both a service delivery and pricing standpoint. A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. Parties responding to this solicitation may submit their quote in accordance with their standard commercial practices (e.g. on company lette rhead, formal quote form, etc.) but must include the following information: 1) Company?s complete mailing and remittance addresses, discounts for prompt payment, if any (e.g. 1% 10 days), anticipated delivery / availability of product / s, the company?s CAGE code, Dun & Bradstreet Number, and Taxpayer ID number. All FAR certifications and representations specified above must also accompany your quote. QUOTES MUST BE RECEIVED NO LATER THAN 3:00 PM, LOCAL TIME 3 October 2002 .
