- Description
- Contractor to provide Community-Based Primary Care services to veterans of the Department of Veterans Affairs Medical Center, Butler, PA, for the period from date of Notice to Proceed until September 30, 2003 plus two (2) One-year renewal option periods. Contractor_s facility to provide services must be physically located in Armstrong County, PA. Pricing is on a _per member per month (PMPM)_ basis and represents an _all inclusive_ reimbursement for routine delivery of primary care. Offerors should quote a monthly capitation rate per patient.
