- Description
- Provide patient follow-up services for patients of the VA Salt Lake Health Care System in accordance with the terms and conditions of the contract from Date of award through 11-30-03 with four one-year options to extend the term of the contract. The contract will be a commercial fixed price contract. This procurement will utilize Simplified Acquisition Procedures in accordance with Commercial Items Part 12 and in accordance with Far Part 15, Contracting by Negotiation.
