- Description
- This is a combined synopsis/solicitation for commercial items in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the ONLY solicitation; a quotation is being requested and a written solicitation WILL NOT be issued. This objective of request for quotation (RFQ), number 02DWEL000033 is to acquire preventative maintenance and/or maintenance services for one or more of the Federal Occupational Health Wellness/fitness sites. There are a total of seven (7) sites. These sites are located throughout Washington, DC (zip codes: 20201, 20001, 20229, 20224, 20591); Maryland (zip code: 20706) and Virginia (zip code: 22202). The Contractor shall provide Preventative Maintenance to all the FOH wellness/fitness sites listed above listed. Preventative Maintenance shall be provided on a bimonthly basis according manufacturers guidelines and include: 1) Cleaning the exterior of all equipment with mild detergent; 2) Adjust and lubricate equipment per manufacture?s guidelines; 3) Inspect for loose, broken or worn out parts; 4) Tighten any loose bolts and replace any missing bolts or fasteners; 5) Vacuum inside of motor compartment and underneath all treadmills; 6) Notify W/F Coordinator of needed repairs or replacement parts and provide a written quote for their repair. The Contractor shall provide all necessary equipment and supplies necessary to complete the preventative maintenance. Solicitation documents, provisions and clauses herein are those in effect through Federal Acquisition Circular 2001-09. The provision at FAR 52.212-1, Instructions to Offerors--Commercial Items (FEB 2002), and the clause at FAR 52.212-4, Contract Terms and Conditions--Commercial Items (MAY 2001), applies to this acquisition. Offerors shall submit a fully executed copy of the provision at FAR 52.212-3, Offerors Representations and Certifications--Commercial Items (JUL 2002) with their offer. The clause at FAR 52.212-5, Contract Terms and Conditions Require to Implement Statutes or Executive Orders--Commercial Items (MAY 2002), applies to this acquisition as well as the following clauses cited therein: FAR 53.222-26, Equal Opportunity (E.O. 11246); FAR 52.222-35, Equal Opportunity for Special Disabled Veterans, Veterans of the Vietnam Era, and Other Eligible Veterans (29 U.S.C. 793); FAR 52-222-36, Affirmative Action for Workers with Disabilities (29 U.S.C. 793); FAR 52.222-37, Employment Reports on Special Disabled Veterans, Veterans of the Vietnam Era, and Other Eligible Veterans (38 U.S.C. 42); FAR 52.225-3, Buy American Act - Supplies (41.U.S.C.) and FAR 52.232-34, Payment by Electronic Funds Transfer--Other than Central Contractors Registration (31 U.S.C. 3332). Firms having the ability to provide the services, as stated above, shall provide clear and comprehensive information supporting its claim, inclusive of references listing contact person(s) and phone number(s); GSA schedule number , if applicable; and pricing.
