- The 99th Contracting Squadron, Nellis AFB, NV is soliciting offers for decontamination equipment for the Mike O?Callaghan Federal Hospital. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is F26600-03-Q-B0002 and will be issued as a request for quotation. The solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 2001-09. This acquisition is set-aside for small business . There are ten (10) line items included in this solicitation. Line Item 0001 is one (1) 50 ft Fire Hose, Part Number: 51155/OR APPROVED EQUAL. Line Item 0002 is one Containment Berm, 18x18x18, Part Number: 51566/OR APPROVED EQUAL. Line Item 0003 is one (1) Wrench Hydrant, Part Number: 51295/OR APPROVED EQUAL. Line Item 0004 is four (4) packs of Water Proof Triage Cards, Part Number: 51087. Line Item 0005 is one (1) Submersible Water Pump 110V, Part Number 51301/OR APPROVED EQUAL. Line Item 0006 is one (1) Patient Elevation Grid, P/N 51286/OR APPROVED EQUAL, Line Item 0007 is one (1) Patient Roller System, Part Number 51563/OR APPROVED EQUAL. Line Item 0008 is one (1) Decon/Triage Shelter, 14x16 feet, Part Number: 51562/OR APPROVED EQUAL. Line Item 0009 is one 1200 Gallon Bladder, Part Number: 51564/OR APPROVED EQUAL. Line Item 0010 is one (1) Water Heater Decon, Part Number 50021/OR APPROVED EQUAL. The manufacturer for Line Item 0001 to Line Item 0010 is the Reeves Group. Delivery will be FOB Destination to Nellis AFB, NV 89191. The North American Industry Classification System (NAICS) is 421450 and the size standard is 500 employees. Award will be made to the lowest priced technically acceptable offer. The provision at FAR 52.211-6, Brand Name or Equal applies to this acquisition. If quoting on an OR EQUAL product, please provide literature and specifications. Failure to do so may result in loss of award. Award will be made to the lowest priced technically acceptable offer. This acquisition is being procured and will be awarded using simplified acquisition in accordance with FAR 13. OFFERORS MUST SUBMIT A COMPLETED COPY OF THE PROVISION AT FAR 52.212-3, OFFEROR REPRESENTATIONS AND CERTIFICATIONS ? COMMERCIAL ITEMS. The clause at FAR 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition.
