Description
This is a combined synopsis/solicitation for commercial services prepared in accordance with format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation is issued as a Request for Quotation (RFQ), solicitation number F05611-03-Q-0717, in accordance with FAR Parts 12 & 13. The North American Industrial Classification System (NAICS) number is 713940 and the business size standard is 500 employees. The proposed acquisition is reserved for small business. LINE ITEM 0001: Provide services to Operate a Water Exercise Program, 1 Nov 02 through 30 Sep 03, eleven months. LINE ITEM 0002: Option year one. LINE ITEM 0003: Option year two. LINE ITEM 0004: Option year three. The FOB for this RFQ is Destination. Quotations received with other than FOB Destination pricing will not be considered. Your proposal should be provided as a monthly amount and a yearly-extended amount for the base and option years. Offerors must comply with all instruction contained in FAR 52.212-1, Instructions to Offerors-Comercial Items, applies to this acquisition and on addenda applies. The provisions at FAR 52.212-2, Evaluation Commercial Items applies to this acquisition. The Government will award a contract resulting from this solicitation to the responsive, responsible offeror whose acceptable offer conforming to the Statement of Work, will be most advantageous to the government, price and other factors considered. Factors considered for award are price and past performance. Price and past performance will be of equal value. Identify past or current contracts (including Federal, State, and local government and private) for efforts similar to the Government requirement, within the last three years. Past performance information should include: company name, address, phone/fax number, POC/e-mail address, and contact number if applicable. The Clause 52.212-3, Contract Terms and Conditions-Commercial Items, applies to this acquisition. The Clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items apply to this acquisition. Additional FAR clauses cited in the clause applicable to this acquisition are: (b) 12,13,14,15,24, (c) 1,2,3,4. Reference (c) 1, of 52.212-5, Wage Determination 1994-2079, Rev 23 applies. Reference (c) 2, Wage equivalence, Recreation Specialist, GS-07, 14.66 per hour. The Clause 52.217-8, Option to Extend Services applies to this acquisition. The Clause 52.217-9, Option to Extend the Terms of the Contract applies to this acquisition. The Clause 52.232-18, Availability of Funds applies to this acquisition. DFARS 252.212-7001, Contract Terms and Conditions Required to Implement Status of Executive Orders applicable to Defense Acquisition of Commercial Items, applies to this acquisition with the inclusion of the following DFARS: 252.204-7004, Required Central Contractor Registration.1. Statement of Work:DESCRIPTION OF SERVICES. Contractor shall provide the services necessary to operate a diversified, professional group exercise program for the USAFA Fitness Center, located in building 5234. This program is governed by and only for authorized users in accordance with AFI 34-266 and 34-262. 1.1. Contractor is responsible for recruiting, training, and scheduling certified instructors who possess as a minimum current national certification(s) such as American Council on Exercise (ACE)-Water Exercise Specialty, United States Water Fitness Association (USWFA), YMCA, or Aquatic Exercise Association (AEA). Instructors must also possess and maintain current national certification in CPR and First Aid from organizations such as American Red Cross or Heart Association. All certifications are at the contractor?s expense. 1.2. Contractor shall present a copy of each instructor?s certification (s) that will be maintained on file in the fitness center. All certifications must be on file prior to instructing any classes. If certifications are not on file, payment will not be received for classes taught. 1.3. Contractor shall be responsible conducting classes during the following days and times: 1.3.1. Monday through Friday, 0600-0800, 1100-1300, 1600-2000 1.3.2. Saturday, 0800-1630 1.3.3. A minimum of 10 and maximum of 20 classes per week 1.3.4. Fitness Center Director reserves the right to change days and times of classes with a two (2) week notice 1.3.5. Fitness Center will provide a quarterly classes schedule at least 30 days prior to start of the next quarter. Contractor will concur or submit recommended changes to schedule for implementation at least 15 days prior to start of quarter 1.4. Contractor shall be responsible for the following in performance of the contract: 1.4.1. Conduct 45-60 minute classes that include a warm-up, main training segment, and cool down 1.4.2. Ensure participants sign-in on the class roster and verify eligibility for the class 1.4.3. Ensure the facility/pool and all equipment is safe for use and all concerns are reported to the fitness center staff 1.4.4. Setup for each class and return all equipment and supplies to its proper location 1.4.5. Ensure the room is clean and presentable before departing 1.4.6. Turn in all rosters to the fitness center staff and filled out correctly 1.4.7. Ensure fitness center policies are followed at all times, to include conduct and safety issues 1.4.8. Be responsible for class format, selection of exercises appropriate for participants, music (tapes or CDs), beginning and ending classes on time 1.4.9. Check in with the front counter at least ten (10) minutes prior to class start time; should the instructor fail to show and the class is canceled, the scheduled payment will be forfeited. 1.4.10. Conduct Weight Management and Get Fit classes in support of the Air Force Fitness Improvement Program (FIP), classes will be identified on the schedule as FIP classes and active duty members in the program have priority; a minimum of one (1) FIP?s will be offered per week 1.5. Cancelled Classes. The fitness center staff must be notified at least 24 hours in advance if a class must be cancelled. In emergency situations, i.e., road conditions, personal emergency), the fitness center staff will be notified immediately. Contractor will only be paid for classes conducted, unless the emergency involves closure of the facility, closure to the base, or the fitness center cancels the class with less than 24 hours notice to the contractor. Contractor will be paid for scheduled classes when the instructor is there, but no participants show up, and the contractor has not been notified of a cancellation at least 24 hours in advance. Instructor must wait at least 15 minutes, and then may leave upon signing the blank roster and notifying the staff. 1.6. Emergencies. Notify the fitness staff, 333-4522 or 333-3531, of any injuries or accidents pertaining to the class. The staff will call 911, and if not available, the instructor is to call 911 for medical assistance. Instructor is also responsible to administer care as deemed appropriate for the circumstance until medical care arrives. 1.7. Invoices. Contractor will invoice for classes taught within the month being invoiced. Invoice must reflect class, quantity, and dates covered by invoice. Invoices must be received by the third working day of the next month and will be paid monthly in arrears. 1.8. Classes. The following is a list of potential classes to be taught. Water Aerobics Aqua Therapy Water Walking Deep Water Aerobics Deep Water Jogging Arthritis Pregnancy & Post-Partum Aqua Step Aqua Kickboxing Senior Fit The Noodle Workout Physical Therapy Water Running Therapeutic Water Exercise Hydro-Fit Shallow Water Exercise Aqua-Tone Quotes must be received NLT 12;00pm Mountain Daylight Savings Time, October 23, 2002, at 10 ABW/LGCB, Operational Contracting Office, 8110 Industrial Drive, Suite 200, USAF Academy, CO 80840-2315.
