Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR 13 and the format in FAR subpart 12.6. This announcement constitutes the ONLY solicitation; quotes are being requested and a separated written solicitation will not be issued. Solicitation Number RFQ 03-REHAB-0001 applies. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-09. The NAICS code is 81311 and the business size standard is $5 million. The National Hansen?s Disease Programs (NHDP) requires the services of a Catholic Priest to provide services for the Gillis Long Center at Carville LA. The contract time period is for 11 months (November 1, 2002 through September 30, 2003) with four one year options. Line Items are 0001 (Basic), 0002 through 0005 (Options 1 through 4, respectively). Prices should be per month to provide the services described below. This requirement is for a fixed price, non-personal services contract. The Statement of Work is as follows: A Catholic Priest shall celebrate 52 Sunday Masses, and Masses for Holy Days of Obligation, and weekly masses on Tuesdays and Wednesdays. Additional Masses and other services will be scheduled as appropriate with mutual agreement of the parties. These services include, but are not limited to sacramental acts for patients and staff on days other than Sunday or Holy Days of Obligation, religious education as needed, and conduct patient and staff visitation on Sundays and weekdays as the need may arise. During the term of this contract, the contractor agrees to perform on behalf of the U.S. Government duties of a Roman Catholic priest for spiritual care of NHDP patients, staff, eligible civilian employees and other beneficiaries per the term of the contract. C. Place of Performance ? Work shall be performed at the Gillis Long Center, Carville LA. Times for masses will be determined by mutual consent between the contractor and the NHDP Project Officer. D. Communication ? The contractor is authorized to meet with Directors, Branch Heads, supervisors and other staff personnel as well as patients and their families in the performance of pastoral responsibilities. The contractor will pursue all administrative duties through the Project Officer and/or Contracting Officer. E. Personal Qualifications ? The contractor shall be a validly ordained Roman Catholic priest in good standing with his diocese or religious order. The Catholic priest shall demonstrate reliability and good relational skills. The contractor will comply with government policies and regulations in regard to identification badges, parking and professional decorum. F. Personal Appearance ? The contractor shall be neat, well groomed and in appropriate clergy clothing (shirt with Roman collar) when in patient care and public areas. G. Government Regulations ? The contractor shall become acquainted with and obey all pertinent NHDP regulations. All motor vehicles operated on the installation by the contractor shall be registered with security and operated according to applicable directives. Smoking by the contractor is prohibited except in designated areas. H. Absences - The contractor shall provide a competent priest substitute with equivalent credentials who will provide Catholic coverage during periods of absences. K. As an independent contractor, all medical, dental and professional liability insurance is the responsibility of the contractor. L. Property ? The Government will provide office space, office furniture, telephone, fax, service, use of a copier and computer hardware and software necessary for use by the contractor. The Government will allow telephone usage, both local and long distance, in support of the services provided under the contract. Within this clause the following clauses apply: 52.222-3 Convict Labor; 52.233-3 Protest after Award; 52.222-21 Prohibition of Segregated Facilities; 52.222-26 Equal Opportunity; 52.222-35 Affirmative Action for Disabled Veterans and Veterans of the Vietnam Era; 52.222-36, Affirmative Action for Workers with Disabilities; 52.222-37 Employment Reports on Disabled Veterans and Veterans of the Vietnam Era; 52.225-13 Restriction on Certain Foreign Purchases; 52.232-34 Payment by Electronic Funds Transfer Other Than Central Contractor Registration; Also included are FAR 52.237-1 Site Visit and 52.237-2 Protection of Government Buildings. Other clauses pertinent to this solicitation are 52.217-8, Option to Extend Services; 52.217-9 Option to Extend the Term of the Contract; 52.232-18, Availability of Funds; 52.252-2, Clauses Incorporated By Reference; Copies of any of the referenced FAR clauses and provisions may be found at http:// www.arnet.gov/far. Quote should consist of price proposed for each item and offerors statement of capabilities to met the requirements described above, with references and/or documentation as appropriate to substantiate proposal. Basis of award will be based on ability to meet the governments requirements, past performance and price. Also include your firm?s prompt payment terms and completed copies of the applicable paragraphs of FAR 52.212-3, and FAR 52.222-48.
