Modification to the Pre-Solicitation Notice is hereby issued to ext end the response date from 15 Oct 02 to 23 Oct 02. If you have any concern or question please feel free to contact me. STATEMENT OF WORK ARMY COMMUNITY SERVICE RELOCATION READINESS PROGRAM C.1 GENERAL Army Community Service (ACS) programs are essential to the life support of soldiers and their families. ACS Relocation Readiness Program provides counseling before, during and after the move. Specialized training for pre/post moves, sponsorship, cross-cult ural adaptation training are also provided. Provide assistance finding: consumer information, locating areas on and off post, resource for special needs, medical and educational information and employment information and operates a Lending Closet. The Lending Closet (LC) is a loan service of basic household utensils designed to assist active duty, reserve service members and DoD civilians and their families until arrival of their own goods or for departing personnel after shipment of goods prior to departure. Requirements for this contract are in accordance with AR 608-1, Army Community Service Center, which mandates all Relocation Readiness Program to operate a Lending Closet for the benefit of military personnel and dependents as they relocate. C-2 CONTRACT SERVICES C.2.1 The contractor shall perform specific services as indicated in Section C 5. Contractor will provide services 30 hours per week, generally during the operating hours of the ACS office (Monday through Friday, 0700-1600) from 1 October 2002 through 30 S eptember 2003). Contract may be renewed (2nd option year, 1 October 2003, through 30 September 2004) at the option of the government subject to the availability of funds. Official established performance work month for this contract would be first through last day of each month. C.2.2 Contractor services are not required on legal holidays (see C 7), during closure of the post and/or the facility for emergency situations, and officially declared training holidays. Contractor will be expected to alter work schedule if necessary duri ng weeks that include a holiday. C.2.3 Contractor will make no private use of any government facilities or other government property in connection with this contract. C.2.4 Contractor will submit to an Installation Records Check (IRC) in accordance with regulation. The IRC includes a screening of past law enforcement and other records, e.g., checks by Criminal Investigation Division, Army Central Registry and Army Alcoh ol and Substance Abuse Program. Contractor will sign an authorization form to initiate this process and contract will be contingent upon satisfactory findings as determined by the Contracting Office Representative and Family Advocacy Program Manager. C.2.5 Services must adhere to standards of practice set forth by the U .S. Army and relevant federal, state, and local law. Contractor must demonstrate sound professional judgment and highest ethical standards in executing contract responsibilities. C.2.6 The Relocation Readiness Program Manager (RRPM) will be the Contracting Officer's Representative (COR) responsible for review of contract requirements and supervision of casework. RRPM will assign work, review case assessments and goals, and monitor ongoing work with families and groups. RRPM will review work on a regular and ongoing basis. C.2.7 Contractor will maintain a neat, well-groomed appearance and workspace at all times. C.3.1 Specialized Work Experience: a. Minimum (1) year experience providing customer support, basic knowledge of suppy operations. b. Work experience could be military, civilian, volunteer. C.3.2 General Experience: Minimum of one (1) year experience in military lifestyle, e.g., prior work experience within a military community service agency, military families or military service. C.3.3 Education: Minimum: High School diploma. Maximum: At least 48 semester credits or an Associate Degree on an Administrative Field. Combination of experience and education will be considered. C.3.4. Related Knowledge, Skills, and Abilities: a. Knowledge of basic supply skills (issuing, receiving, turn-in of property). b. Working knowledge of local military and civilian resources to support families and ability to work cooperatively with referral agencies to connect military families with needed services. c. Familiarity with ACS Relocation Readiness Program; ability to exercise sound judgment and perform professionally and ethically IAW regulation and local ECS policies and procedures. d. Demonstrated ability to develop trusting relationships and work effectively with individuals and families from diverse racial, ethnic, and socioeconomic backgrounds, sometimes in crisis situations. Contractor must be able to operate independently to as sess client and community needs and develop, execute, and evaluate plans to address those needs. e. Proficiency in oral and written communication skills (english and spanish). Proficiency in basic Microsoft / Office computer skills. f. Physical ability to work with mid to heavy supplies in a safety manner. g. Contractor will hold a current motor vehicle license and use his/her own vehicle for business conducted within Fort Buchanan premises. Be able to acquire a military vehicle license and operate military vehicles as needed. 2 C-3 CREDENTIALS C-4 COMPENSATION C.4.1 For satisfactory completion of the services under this contract, the contractor shall be paid monthly, at the end of the month, until expiration/termination of this contract. C.4.2 Contractor shall furnish the Contracting Officer's Representation (COR) his/her invoice at the end of each work month. Nonperformance hours for absences other than identified in C.2.2 above shall be shown on invoice. Dividing monthly rate of pay by 1 72 and multiplying by number of nonperformance hours will make Reduction(s) from monthly pay for each nonperformance hour. C-5 SPECIFIC REQUIREMENTS C.5.1 Maintain LC items in clean and serviceable condition. Ensure dish packs are assembled and available for issuance at all times. Keep LC area clean and neat. Assist clients who wish to borrow/return items from/to the LC. Prepare statistics covering LC clients (new/cleared) and submits to the RRPM weekly each Friday. As a minimum, an annual inventory of LC items shall be submitted by the contractor to the RRPM NLT 30 days after the beginning of the contract. LC items shall be issued for a period of 30 da ys. Extensions shall be granted for an additional 30 days if household goods are not yet available. For periods longer than 60 days, verification of proposed arrival date from the transportation office is required. The RRPM or ACSO authorizes exceptions. V erifies that personnel must present a current ID card and a copy of sponsor?s unit assignment orders and complete a LC Issue Receipt. NOTE: Personnel possessing valid ID card who experience a disaster (fire, theft, water damage, etc.) are eligible for loan of LC items. Contractor will require the sponsor/family member read and sign the receipt. The Contractor shall print any items borrowed that are not preprinted on the receipt on the receipt. Contractor after issuing the items must enter his/her name in th e block, ?Signature of ACS representative.? Do not use initials. A copy of the receipt is given to the sponsor/family member. The copy of the sponsors? orders is attached to the original receipt and placed in the LC file alphabetically. C.5.2 Contractor will issue quantities of LC items based upon the total number of family members. Records condition of items on the receipt at the time of issue (if other than normal condition is apparent ). Contractor will remind users of the LC of their financial responsibilities for borrowed items. In the event of lost or damaged items, the client will be required to replace the item with an equivalent new item. Each item returned is to be checked against the original receipt. Items will ONLY be accepted in clean and good condition. (Clients shall not clear the LC until all items are returned and accepted or replacement is made). Once all items are accepted, the Contractor shall sign and date the sponsor orders and return to the client. Receipt is then destroyed upon receiving the property. C.5.3 Performs one or mote of the following duties: receives visitors and phone calls, and refers them to the appropriate person(s), office; answers routine inquires; schedules appointments with the aid of an appointment calendar; date-stamps incoming mail ; reviews, sorts and distributes mail to appropiate person(s), office; maintains a follow-up system to ensure timely completion; creates/maintains filing systems and files a variety of documents; orders/maintains office supplies and other materials; provid es office supplies and materials when requested; updates/maintains any type of manual and or automated log and operates fax machines and copy machines. 3 4 C-6 REMOVAL OF CONTRACT PERSONNEL C.6.1 The Government will have the right to cause the contractor to replace any individual, functioning as a supervisor or employee, who is determined by the Government to be a security risk, under the influence of alcohol or drugs, or is physically or men tally impaired to the extent that they cannot perform the tasks established by the SOW. Such determination to be made within the sole discretion of the Contracting Officer or his/her designee. Such determination will not relieve contractor of meeting the performance requirements of this contract. C-7 LEGAL PUBLIC HOLIDAYS C. 7.1 Contractor shall not perform on legal public holidays. They are as follow: January 1 New Year's Day January 15 Dr. Martin Luther King's Birthday Third Monday in February -President's Day Last Monday in May -Memorial Day July 4 Independence Day First Monday in September -Labor Day Second Monday in October -Columbus Day November 11 Veteran's Day Fourth Thursday in November -Thanksgiving Day December 25 -Christmas Day C.5.4 Assist and accompany the RRPMA to different activities as required. Activities such as but not limited to: Newcomers Briefing, Newcomers Tour, Levy briefing, Farmers Market Tour, Hasta Luego ceremony and any other activity as deemed necessary. Respon sible for conducting activities and or tours alone in the absence of the RRPM or when requested to. C.5.5 Uses office automation equipment and software to a limited degree. Contractor performs the following duties using typical automation equipment: type labels, envelopes and various forms, print messages from electronic mail. Contractor uses software su ch as electronic mail to receive and transmit messages and uses database management software to establish and maintain databases. Also, operates electric typewriter, office Fax machine and office copying machine, providing services as requested. A qualifie d typist is not required. C.5.6 Provides assistance to higher level employees. Assistance may include performing one or more of the following duties: gathering data for reports, studies, etc, referring to dictionaries, manuals, and other guides and running errands inside/outside of fice unit. Assistance should be provided under the approval of the RRPM and without interfering with primary duties of the Contractor. C.5.7 Performs other duties as assigned. C-8 GOVERNMENT FUNISHED PROPERTY AND SERVICES C.8.1 The Government will furnish office space in Building 1019, Fort Buchanan, P.R. This includes a desk, a chair, filing cabinet, access to a computer and copier, and all office supplies necessary to comply with this SOW. 5 C.8.2 The Gove rnment will furnish all Army Regulations, forms, and resource files necessary to comply with this SOW. C.8.3 The Government will furnish fire protection, telephone, utilities, custodial and security services to the extent that they are available to ACS, Building 1019, Fort Buchanan, P. R. C.8.4 The Government will furnish postal and installation distribution services for all official items. C.8.5 The government property made available under this contract shall be for use only in the performance of the contract C.8.6 The Government will not be responsible in any way for damage to the contractor?s supplies, materials, equipment, or the contractor?s employees? personal belongings due to fire, theft, accident, or other disaster. C.8.7 Contractor will be eligible to participate in professional training deemed by COR to be in the best interest of the government as funding allows. Training must be approved by COR. C.9.2 Contractor must provide and maintain a cellular phone for communication with the office while in transit or on authorized trips outside Ft Buchanan. Cellular phone is required for purposes of personal safety and professional accountability. C.9.3 Contractor is not an employee of the federal government. Contractor will be responsible for all income taxes required by law. C-9 CONTRACTOR FURNISHED ITEMS C.9.1 Contractor will furnish his/her own transportation. It will be necessary for the contractor to use his/her own vehic1e to coordinate tasks/actions with military and civilian /agencies and organizations within USARSO, Puerto Rico, theater of operation s. Use of government vehicle for trips to Roosevelt Roads Naval Station and environs is at the discretion of COR and dependent on availability of vehicle.
