Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a WRITTEN SOLICITATION WILL NOT BE ISSUED. This solicitation/synopsis is being issued as RFQ 60605-0003-3. This solicitation/synopsis and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2001-09. This is a requirement for a Contractor to provide services of a Jewish Rabbi to inmates housed at the Metropolitan Detention Center (MDCLA), 535 No. Alameda Street, Los Angeles, CA 90012. STATEMENT OF WORK: The Contractor shall provide Jewish religious services, visitation, spiritual guidance and counseling to the inmates at the Metropolitan Detention Center (MDC) located at 535 North Alameda Street, Los Angeles, CA 90012. All duties shall be performed in accordance with standards and methods generally accepted within the provider's field of expertise. In his/her absence, the contractor shall make arrangements for services to continue. SCHEDULE: Under normal circumstances, services are required for ONE 3-HOUR SESSIONS PER WEEK, and schedule shall be coordinated with the MDCLA Chaplain or his designee. Schedule is subject to change as the needs of the institution vary. Schedule may be adjusted to meet the needs of the institution at the sole discretion of the Government with 24-hour advance notice. The contractor/provider shall visit the Special Housing Unit, under direct visual monitoring by the Government. SUPPLIES: All supplies that the contractor/provider may routinely need shall be provided by the government, subject to prior approval of the appropriate Department's Approving Official/Cost Center Manager. Those supplies include, but are not limited to materials needed to perform the services covered under the contract. Any records generated in the performance of this contract remain the property of the Government. All religious materials and supplies shall be subject to the approval of MDCLA Chaplain. CONTRACT PERFORMANCE PERIOD: The anticipated period of performance will consist of one base period from date of award (approx. November 1, 2002) thru September 30, 2003, followed by up to four (4) option year periods, each beginning on October 1 of each subsequent year. The entire contract period will not exceed five (5) years. The government is not obligated to exercise any or all of the option periods. QUALIFICATION: Must be a certified Rabbi who has been endorsed by the Southern California Board of Rabbis. Documentation of credentials and/or license must be submitted with quotation. The contractor must adhere to all policies and regulations prescribed by the Federal Bureau of Prisons. Therefore, prior to commencing the contract work, he/she shall attend the orientation class program given at the work location prior to start of service. Reimbursement for the time spent in the institution security orientation shall be at a contract rate. This is a contractual arrangement and not a personnel appointment; payment is based on the provision of an end product or the accomplishment of a specific result. The contractor will not be subject to Government supervision, except for security related matters. However, contractor performance will be closely monitored. Payments under the contract will not have provisions for state, municipal, or federal withholding taxes, nor will payments be subject to the Federal Insurance Contributions Act. It will be the contractor's responsibility to report all earnings to the appropriate revenue service for tax purposes. (Note: Federal agencies are required to report all payments to a contractor to the Internal Revenue Service.) The contractor is not entitled to unemployment compensation benefits under the Social Security Act, as amended, by virtue of performance under the contract. The contractor agrees to comply with the Privacy Act of 1974 and the rules and regulations issued pursuant to the Act in the performance of the services required by the contract. SECURITY ISSUES: Any individual who provides service to the BOP must submit to, at a minimum, the following security investigations: National Crime Information Center (NCIC) check, fingerprint check, name check, employment history check and Urinalysis. FAR Provision at 52.212-1, Instruction to Offerors Commercial Items, applies to this acquisition and hereby incorporated by reference. FAR Clause 52.212-2 applies and is incorporated by reference. The Government intends to make a single award to the offeror offering the best value to the Government, price and other factors considered. The other factors include Technical Proposal, Experience and Past Performance. A copy of the provision at 52.212-3, Offeror Representations and Certifications Commercial Items must be included with offers. FAR Clause at 52.212-4, Contract Terms and Conditions Commercial Items, applies to this acquisition. FAR Clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Commercial Items, applies to this acquisition. SUBMISSION OF QUOTATION: On letterhead or business stationery, the following information is required: The unit price of each session, extended to the total period price (Base Period : Date of award to 09/30/03: 48 sessions @ $______ per session, total $ ______ for base period; Option Period One: 52 sessions @ $ ______ per session, total $ ______ : Option Period Two: 52 sessions @ $ _______ per session, total $ ________; Option Period Three 52 sessions @ $ _______ per sessions, total $________, Option Period Four: 52 sessions @ $_______ per session, total $_________.)
