Description
NA 10. This is a combined/solicitation/synopsis for commercial items prepared in accordance with FAR 13 and the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. Solicitation No. N00244-03-T-0063 is issued as a Request for Quotations (RFQ). All references of ?offers? are understood to be ?Quotations?. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 01-07 and Defense Federal Acquisition Regulation Supplement Revisions 20020531 dated 31 May 2002. The NAICS Code 621112 Offices of Physicians, Mental Health Specialist and the small business size standard is $7.5 million. Services required by Navy Central Adjudication Facility to hav e employees evaluated to determine whether any medical, psychological, psychiatric or emotional condition exists which could effect the individual?s judgment or reliability for security clearance purposes. A written report is required with the following: Applicable medical consideration should include the following for mental/emotional conditions: a. Does the individual currently, or by history have any illness mental condition, or dysfunctional behavior which, may cause a material defect in his/her ability and/or willingness to properly safeguard classified information or perform sensitive duties? b. Does the individual exhibit any illness, mental condition or dysfunctional behavior even though there is no confirming medical history? c. If there was a prior diagnosed mental condition, is it of a nature which has a high risk of recurrence even though the individual currently manifests no symptoms of mental or emotional condition? d. If any of the above conditions exists, provide the following information in layman terms: (1) DMS-IV multiaxial evaluation which include diagnosis, prognosis and a Global Assessment of Functioning. (2) Date of onset of the condition. Is the behavior likely to continue? Is it controlled by medication? What type of medication and dosage? Is the medication being taken as prescribed? Is the individual experiencing any side effects from the prescribed medication? If yes, what are the side effects. (3) Has appropriate medical advice relating to treatment of a diagnosed illness, mental condition or dysfunctional behavior been followed? Is treatment or counseling on-going at this time, or is it recommended? (4) Has the cause of the material defect been cured? Is the condition in remission? What is the probility of recurr ence? If the issue involves sexual conduct, provide the following information (in addition to the above): a. History or extent of the conduct; is it likely to recur? If so, under what circumstances? Is the individual aware of the seriousness of the conduct? If the issue involves substance abuse, provide the following information (in addition to the above): a. History or extent of involvement with substance abuse; is it likely to recur? If so, under what circumstances? Is the individual now or has he/she been addicted or dependent upon the substance? b. Do you feel the individual is likely to participate in such conduct in the future? Can the individual?s substance abuse be a basis for vulnerability to blackmail, pressure or coercion? c. Has the individual?s physical or mental health deteriorated as a result of substance abuse? d. What are the long and short term effect o n the individual?s behavior, if any, of substance(s) abused? Provide a formal DSM-IV multiaxial evaluation which include diagnosis, prognosis and a global assessment of functioning. Also a statement as whether any diagnosed mental condition or dysfunctional behavior identified may cause a material defect in the individual?s ability and/or willingness to properly follow orders, safeguard classified information or perform sensitive duties. The above factors were extracted from SECNAVINST 5510.30A and may not be all inclusive and you need not limit the evaluation or examination to the above factors. No copy of the evaluation will be provided to the individuals. Reports will be mailed to Corona, CA. There is no way to predict the timing of the exams. Service will be paid on a case-by-case basis. Services required within 75-miles from the Corona site. Anticipated number of ex ams is 15 within 12 months. Period of performance, date of order thru 30 Sep 03. Firm Fixed Price, Indefinite Delivery, Indefinite Quantity contract anticipated with a minimum of one report and a maximum of 15 reports. FOB destination Corona, CA. The following FAR provisions are applicable to this RFP ? FAR 52.212-1, Instructions to Offerors ? Commercial Items; Addendum to FAR 52.212-1, Paragraph (b)(5): Offers shall provide an express warranty which at minimum shall be the same warranty terms, including offers of extended warranties, offered to the general public. Express warranties shall be included in the contract. FAR 52.212-4, Contract Terms and Conditions Required to Implement Statutes for Executive Orders ? Commercial Items applies as well as the following addendum clauses: FAR 219-1, Small Business Program Representations; FAR 52.211-5, Material Requirements. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statues or Executive Orders-Commercial Items applies with the following applicable clauses for paragraph (b): FAR 52.222-37, Employment Reports on Disabled Veterans and Veterans of the Vietnam Era.;52.222-41, Service Contract Act of 1965. DFARS 252.212-7001, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Applicable to Defense Acquisition of Commercial Items, applies with the following clauses applicable for paragraph (b): DFARS 252.225-7001, Buy American Act and Balance of Payment Program; and DFARS 252.225-7012, Preference for Certain Domestic Commodities; and addendum provisions DFARS for Certain Domestic Commodities. OFFERORS ARE REQUIRED TO COMPLETE AND INCLUDE A COPY OF THE FOLLOWING PROVISIONS WITH THEIR PROPOSALS: FAR 52.212-3, Offeror Representation and Cert ifications ? Commercial Items.
