- W--Lease of blood analysis equipment. This is a combined synopsis/solicitation for the lease of commercial blood analysis equipment prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. The announcement constitutes the only solicitation; proposals are being requested, and a written solicitation will not be issued. This solicitation is issued as a request for proposal (RFP) N00102-03-R-1306. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-08 with changes effective 7/29/02 and DFARS current as of 7/30/02, inclusive of DCN 20020730 effective 7/30/02. The blood analysis equipment shall be Beckman Coulter (brand name mandatory). Lease of two Coulter AcT 5 Diff CAP Piercing Blood Analysis machines with data management terminals and printers for one year with four option years as follows: Item 0001 - Lease of one Coulter AcT 5 Diff CAP Piercing Blood Analysis machine with data management terminals and printers, P/N 6605705, on a Cost Per Test Lease basis (15 tests per day, 5 days per week, 52 weeks per year) for Laboratory, Branch Medical Clinic, Naval Air Station Brunswick, Brunswick, ME. The lease shall include equipment, parts, service, reagents, and training. The initial lease period shall be from date of installation through 30 September 2003. Item 0002 - Lease of one Coulter AcT 5 Diff CAP Piercing Blood Analysis machine with data management terminals and printers, P/N 6605705, on a Cost Per Test Lease basis (33 tests per day, 5 days per week, 52 weeks per year) for Laboratory, Naval Ambulatory Care Center, Portsmouth Naval Shipyard, Portsmouth, NH. The lease agreement shall include equipm ent, parts, service, reagents, and training. The initial lease period shall be from date of installation through 30 September 2003. Items 0003, 0005, 0007, and 0009 are option years to extend the lease of equipment identified in Item 0001; Items 0004, 0006, 0008, and 0010 are option years to extend the lease of equipment identified in Item 0002. Each option item is for a period of 12 months beginning 01 October through 30 September and will be exercised by modification no later than 15 September of each year. Offers shall include monthly lease rates for each item. The government shall provide a 30-day notice for termination of the lease. There shall be no penalty for terminating the lease. Delivery of equipment shall be FOB destination (Brunswick, ME for Item 0001 and Portsmouth, NH for Item 0002). Delivery, installation, and training shall be completed within 30 days a fter receipt of contract. The provisions at 52.212-1, Instructions to Offerors --Commercial, applies to this acquisition. Offerors are requested to include a completed copy of the provision at 52.212-3 and 252-212-7000, Offeror Representations and Certifications--Commercial Items, with offers. The following clauses/provisions apply to this acquisition: 52.212-4, 52.212-5 (including 52.222-21, 52.222-22, 52.222-25, 52.222-26, 52.222-35, 52.222-36, 52.222-37, and 52.225-3), and 252.212-7001. Original offers are due no later than 05 November 2002 at 3:30 PM local time at Portsmouth Naval Shipyard, Contracting Division, Building 153, 6th Floor, Code 531.1X, Portsmouth, NH 03801-2590.
