Description

This is a combined synopsis/solicitation for commercial items or services prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation is issued as a Request for Quotation (RFQ), solicitation number F05611-03-Q-0500, in accordance with FAR Parts 12 & 13. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 01-03. The North American Industrial Classification System (NAICS) number is 315221 and the business size standard is 500 employees. The proposed acquisition is reserved for small businesses. Women owned and disabled veteran owned small businesses are encouraged to submit quotes. LINE ITEM 0001: T-Shirts, Short-Sleeved, Personalized for the Class of 2006, USAFA, with Cadet?s Name, Colors: White/Navy, Round Neck, 100 percent Preshrunk Cotton. Has to be manufactured IAW Buy America Act and IAW USAFA Specifications PD 34 TRW/SDCU-174, dated 21 August 2002. Quantity: 4,380 each of assorted sizes from Small to XXLarge. Unit Price $__________ Total Price $________ Delivery Date: _______________LINE ITEM 0002: T-Shirts, Long-Sleeved, Personalized for the Class of 2006, USAFA, with Cadet?s Name, Colors: White/Navy, Round Neck, 100 percent Preshrunk Cotton. Has to be manufactured IAW Buy America Act and IAW USAFA Specifications PD 34 TRW/SDCU-175, dated 24 July 2002. Quantity: 2190 each of assorted sizes from Small to XXLarge. Packaging and marking requirement: Unit pack of 6 pair per box. Each box must have affixed a bar code label with a 10 MSG/LGLC approved stock number, brief description and size. A sample bar code label will be provided. Bar code label must be approved prior to shipment. First Article is required for this RFQ. The FOB for this RFQ is Destination. Quotations received with other than FOB Destination pricing will not be considered. Offerors must comply with all instructions contained in FAR 52.212-1, Instructions to Offerors--Commercial Items, applies to this acquisition and the following addenda applies: The provisions at 52-212-2, Evaluation--Commercial Items applies to this acquisition. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the government. The following factors shall be used to evaluate offers: Price and delivery time. Offers shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications--Commercial Items, with its offer (Attachment One). The clause 52.212-4, Contract Terms and Conditions--Commercial Items, applies to this acquisition. The clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders--Commercial Items applies to this acquisition. Additional FAR clauses cited in the clause applicable to this acquisition are: (b)12, 13, 14, 15, 16, 24 DFARS 252.212-7001, Contract terms and conditions required to implement statues of Executive Orders applicable to Defense Acquisitions of Commercial Items, applies to this acquisition with the inclusion of the following DFARS: 252.204-7004, Required Central Contractor Registration, 252.225-7000, Buy American Act - Balance of Payments Program Certificate, 252.225-7001 Buy American Act and Balance of Payments Program, and 52.209-4 First Article Approval?Government Testing.
