- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. (ii) The reference number is FM448822280001 and the solicitation is issued as a request for quotation (RFQ). (iii) This solicitation document incorporates provisions and clauses in effect through Federal Acquisition Circular 01-08. (iv) This solicitation is 100% set aside for small business concerns. The associated NAICS code is 339114, and the small business size standard is 500 employees. (v) This requirement consists of six line items. (vi) Line Item 0001: Fully automatic washer/disinfector. See Specifications; PN 8666SDATAVL; Quantity: 1 each. Line Item 0002: Manual Transfer/Load Trolley (with and without drain assembly); PN KGYD010345; Quantity: 2 each. Line Item 0003: Wash Cart, 2-Level, Manual and Auto Operation; PN KGY498-6762-05; Quantity: 2 each. Line Item 0004: Dental Cassette Rack (two racks fit on each level, each rack accommodates 10 cassettes; PN KY010012; Quantity: 2 each. Line Item 0005: Recess trim for one washer (one side of wall); PN KY980115; Quantity: 2 each. Line Item 0006: Installation and Removal, Includes: A site walkthrough; utility requirements review; uncrating, moving and setting the equipment in place; reassembly (if necessary); final utility connections; and an equipment operational checkout with a certified report. In addition, existing Equipment will be removed and relocated to the facility?s loading dock; PN I/R-8666; Quantity: 1 lump sum. Manufacturer: Getinge Castle, or equal. FOB shall be destination, and shipping should be included in the unit price. (vii) An optional site visit is planned for Thursday, 31 Oct 2002, beginning at 9:00 AM EST. (xi) The provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, applies to this solicitation. The contractor shall return a completed copy of this provision with its quotation. A copy of the provision may be attained from http://farsite.hill.af.mil. (xii) The clause at 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this acquisition. Paragraph (o) is changed to read: The contractor shall provide all standard commercial warranties to the government (xiii) The clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items, applies to this acquisition. The following FAR clauses identified at paragraph b of FAR 52.212-5 are considered checked and are applicable to this acquisition: (11-16), (19i), (19iii), (21), (24). (xiv) Award can only be made to contractors registered in the Central Contractor Registration web page, http://www.ccr.gov. (xv) The Defense Priorities and Allocations System (DPAS) is not applicable to this acquisition. (xvi) Commerce Business Daily numbered note 1 applies to this acquisition.
